摘要
目的:探讨慢性呼吸系统疾病失能老人长期照护需求。方法:对3家医院132例慢性呼吸系统疾病失能老人的长期照护需求进行调查,并对其中15例失能老人进行半结构式访谈。结果:量性研究结果显示,慢性呼吸系统疾病失能老人的长期照护需求呈现明显的疾病相关特征,以呼吸功能康复相关的专业健康指导需求最为强烈;影响照护需求的因素包括失能程度、医疗负担、医保报销及呼吸/心脏功能。质性研究提炼出3个主题群:照护内容以呼吸功能康复相关的专业健康指导为主;由于需要长期肺功能锻炼,照护方式倾向于居家养老;希望照护者是专业医护人员且愿意付费。结论:慢性呼吸系统疾病的失能老人对长期照护需求强烈且呈现明显的疾病相关特征,照护需求的影响因素多元化。
Objective:To explore the long-term care needs of disabled elderly with chronic respiratory disease.Methods:The long-term care needs of 132 disabled elderly patients with chronic respiratory diseases in 3 hospitals were investigated by using self-design questionnaire.In addition,15 disabled elderly patients were interviewed by semi-structured method.Results:The quantitative research showed that the long-term care needs of the disabled elderly with chronic respiratory disease showed obvious disease-related characteristics,with the most strong demand for professional health guidance related to respiratory rehabilitation.Factors influencing demand for care include degree of disability,medical burden,reimbursement,and respiratory/heart function.Three theme groups were extracted from the qualitative research,which were the mainly nursing care contents with professional health guidance related to respiratory function rehabilitation,including home-based care module because of the long-term need for lung function exercise,wishing the professional caregivers and willing to pay.Conclusions:The disabled elderly with chronic respiratory disease had a strong need for long-term care with disease-related characteristics.The factors influencing the need for care were diversified.
