非营养性吸吮联合俯卧位重力喂养对极低出生体重儿生长发育及胃肠道功能的影响 被引量:10

Effects of non-nutritive sucking combined with gravity feeding in prone position on growth and development and gastrointestinal function of very low birth weight infants

摘要 目的分析非营养性吸吮联合俯卧位重力喂养对极低出生体重儿生长发育及胃肠道功能的影响。方法选取收治的极低出生体重儿80例,分为俯卧位重力喂养组(俯卧位组)40例,俯卧位重力喂养联合非营养性吸吮组(联合组)40例。比较2组新生儿出生14 d时的体重、身长、头围、喂养不耐受、胃液pH值、单位时间内肠鸣音次数以及胎粪排尽时间。结果出生14 d时,2组新生儿体质量、身长和头围均高于出生时,且联合组上述指标高于俯卧位组,联合组新生儿喂养不耐受发生率低于俯卧位组,差异均有统计学意义(P<0.05);胃液pH值俯卧位组高于出生时,联合组低于出生时及俯卧位组,差异均有统计学意义(P<0.05);2组新生儿单位时间内肠鸣音次数高于出生时,且联合组高于俯卧位组,联合组新生儿胎粪排尽时间少于俯卧位组,差异均有统计学意义(P<0.05)。结论俯卧位重力喂养联合非营养性吸吮,有利于极低出生体重儿的生长发育和改善胃肠道功能,并且可减少喂养不耐受情况的发生。 Objective To analyze the effect of non-nutritive sucking combined with gravity feeding in prone position on growth and development and gastrointestinal function of very low birth weight infants.Methods 80 cases of very low birth weight infants were selected and randomly divided into two groups,40 cases in each group.One group was fed in the prone position by gravity feeding and set as the prone position group,and the other group was fed on the basis of non-nutritive sucking and set as the combined group.The growth and development(weight,length and head circumference)of newborns at 14 days after birth were compared between the two groups.The feeding intolerance and gastrointestinal function(PH value of gastric juice,number of bowel sounds per unit time,time of meconium defecation)were compared at the same time.Results At 14 days of birth,the weight,length,and head circumference of the two groups of newborns were higher than those at birth,and the above-mentioned indexes of the newborns in the combined group were higher than those in the prone position group,the differences were statistically significant(P<0.05).The incidence of feeding intolerance in the combined group was lower than that in the prone position group,and the difference was statistically significant(P<0.05).The PH value of gastric juice in the prone position group was higher than that at birth,which in the combined group was lower than that at birth,and the combined group was lower than that in the prone position group,the difference was statistically significant(P<0.05).The number of bowel sounds per unit time in the two groups of newborns was higher than that at birth,and the combined group was higher than the prone position group,the difference was statistically significant(P<0.05).The neonatal fecal depletion time of the combined group was less than that of the lying position group,and the difference was statistically significant(P<0.05).Conclusion For very low birth weight infants,the prone gravity feeding combined with non-nutritive sucking is beneficial to the growth and development of newborns and improves gastrointestinal function,and it can reduce the occurrence of feeding intolerance.
