摘要
目的观察共享决策支持下家长参与护理对新生儿重症监护病房(NICU)早产儿母乳喂养率、生长发育的影响。方法选择本院2018年1月至2019年9月出生的114例早产儿作为研究对象,按随机数字表法分为2组,各57例。对照组行常规护理干预,观察组行共享决策支持下家长参与护理干预。比较干预结束后3月时,母乳喂养指标(母乳喂养率、母乳喂养耐受率);记录两组早产儿6月龄时、12月龄时体格生长指标(体重、身长、头围)、智力行为指标[贝利婴幼儿发展量表(BSID)]水平。结果干预结束后3月时,观察组各母乳喂养指标水平高于对照组(P<0.05);6月龄、12月龄时,观察组早产儿各体格生长水平、BSID评分均高于对照组(P均<0.05)。结论共享决策支持下家长参与护理能显著提高NICU早产儿母乳喂养率,改善早产儿生长发育状况。
Objective To observe the effects of nursing with parental participation under shared decision support on breastfeeding rate and growth and development of premature infants in a neonatal intensive care unit(NICU).Methods 114 premature infants born in the hospital from January 2018 to September 2019 were selected as subjects.According to the random number table method,they were divided into two groups with 57 cases in each group.The control group was given routine nursing intervention,and the observation group was given nursing intervention with parental participation under shared decision support.The breastfeeding indexes(breastfeeding rate,breastfeeding tolerance rate)were compared at three months after the end of intervention.The physical growth indicators(body weight,body length,head circumference)and intelligence behavior index[Bayley Scales of Infant Development(BSID)]were recorded in the two groups of premature infants at six months and twelve months of age.Results At three months after the intervention,the breastfeeding indexes in the observation group were higher than those in control group(P<0.05).At six months and twelve months of age,the physical growth levels and BSID score in observation group were higher than those in the control group(all P<0.05).Conclusion Nursing with parental participation under shared decision support can significantly enhance the breastfeeding rate and improve the growth and development of premature infants in NICU.
