老年脑萎缩伴随精神障碍患者的颅脑CT检查影像学分析

摘要 目的:讨论颅脑CT检查老年脑萎缩伴随精神障碍疾病的价值意义。方法:将本院2016年6月~2019年6月收治的老年脑萎缩患者103例当作观察对象,将伴有精神障碍患者46例当作研究组,将未伴有精神障碍患者57例当作对照组;两组均行颅脑CT检查,比较两组影像学征象特征。结果:对照组的脑白质CT值、脑沟宽度、侧裂池宽度、侧脑室宽度等线性测量影像学指标均值优于研究组,P<0.05。研究组脑室扩张与颅内脑梗死病灶及脑白质脱髓鞘发生率高于对照组,P<0.05。脑萎缩合并精神障碍的46例患者中,有65.22%脑皮质型脑萎缩、28.26%脑白质型脑萎缩、32.61%局部脑萎缩、13.04%小脑与脑干萎缩,一侧大脑半球萎缩47.83%。结论:颅脑CT检查能够明确辨别出老年脑萎缩患者并发随精神障碍疾病的危险因素,能够为临床方案治疗与预后评估提供价值依据。 Objective:To discuss the value and significance of brain CT examination in elderly patients with mental atrophy and mental disorders.Methods:103 cases of elderly patients with cerebral atrophy treated in our hospital from June 2016 to June 2019 were used as observation objects,46 patients with mental disorders were regarded as a research group,and 57 patients without mental disorders were treated as a control group;both groups underwent cranial CT examination,and compared the imaging features of the two groups.Results:The average values of linear measurement imaging indexes such as CT value,width of sulcus,width of lateral fissure pool,width of lateral ventricle in control group were better than those in study group,P<0.05.The incidence of ventricular dilation,intracranial cerebral infarction lesions and white matter demyelination in the study group was higher than that in the control group,P<0.05.Among 46 patients with brain atrophy and mental disorders,65.22%had cerebral cortical atrophy,28.26%had white matter-type atrophy,32.61%had local atrophy,13.04%had cerebellar and brain stem atrophy,and one side had hemisphere atrophy.Conclusion:Cranial CT examination can clearly identify the risk factors of mental disorders in elderly patients with brain atrophy,and can provide a valuable basis for the quality and prognosis of clinical programs.
