摘要
目的:探讨腹腔镜手术在急性嵌顿性腹外疝治疗中的应用价值。方法:回顾性分析医院急诊外科收治的急性嵌顿性腹外疝病例63例(随访55例),依据手术方式分为腹腔镜手术组(36例)与开放手术组(27例),观察两组患者的围手术期指标、术后并发症指标和随访预后指标的差异。结果:腹腔镜手术组和开放手术组患者在手术时间、术后消化道功能恢复时间、住院时间、住院费用、术后并发症、局部硬块和异物感以及需二期疝修补比例等方面差异均具有统计学意义(P<0.05);而两组患者在术中出血量和腹股沟区慢性疼痛等方面差异无统计学意义(P>0.05)。结论:腹腔镜手术治疗急性嵌顿性腹外疝可降低腹腔内病变的漏诊率,具有创伤小、痛苦轻、术后恢复快和术后并发症少等优点,而且提高了一期疝修补的成功率,在临床上有较大应用价值。
Objective:To investigate the value of laparoscopic surgery in the treatment of acute incarcerated external abdominal hernia.Methods:A retrospective analysis of 63 cases of acute incarcerated abdominal hernia treated in hospital for emergency surgery(follow-up of 55 cases)was divided into laparoscopic surgery group(36 cases)and open surgery group(27 cases)according to the surgical method.Observe the differences in perioperative indicators,postoperative complications indicators and follow-up prognosis indicators of the two groups of patients.Results:In the laparoscopic surgery group and the open surgery group,the operation time,postoperative gastrointestinal function recovery time,and hospital stay,hospitalization costs,postoperative complications,local lump and foreign body sensation and the ratio of secondary hernia repair were statistically significant(P<0.05);the intraoperative bleeding volume of the two groups and chronic pain in the groin area were not significantly different(P>0.05).Conclusion:Laparoscopic surgery for acute incarcerated abdominal hernia reduces the rate of missed diagnosis of intra-abdominal lesions,has the advantages of less trauma,less pain,faster postoperative recovery and fewer postoperative complications,and improves the success rate of first-stage hernia repair.It has great application value in clinic.
