摘要
目的:研究以加速康复外科(ERAS)理念为指导的综合干预对剖宫产初产妇母乳喂养效果的影响。方法:将自2016年1月至2018年3月于深圳市人民医院龙华分院行剖宫产的初产妇120例作为研究对象。通过随机抽签法将其均分成研究组与对照组,对照组行传统围术期护理干预,研究组则进行以ERAS理念为指导的综合干预。分别比较两组母乳喂养情况、干预前后心理状况、产妇满意度情况。结果:研究组产后泌乳始动时间短于对照组,术后24 h泌乳量、术后48 h母乳喂养率显著高于对照组,差异均有统计学意义(均 P<0.05)。干预后研究组产妇焦虑自评量表(SAS)、抑郁自评量表(SDS)评分低于对照组,产妇各项满意度指标评分高于对照组,差异均有统计学意义(均 P<0.05)。 结论:以ERAS理念为指导的综合干预可显著提高剖宫产初产妇母乳喂养效果,缓解其心理负性情绪,提高产妇满意度。
Objective To study the influence of comprehensive intervention guided by the concept of enhanced recovery after surgery(ERAS)on breast-feeding effect of cesarean delivery primipara.Methods A total of 120 primiparas who underwent cesarean section in Longhua Branch of Shenzhen People's Hospital from January 2016 to March 2018 were studied.They were divided into study group and control group by random lottery.The control group received traditional perioperative nursing intervention,while the study group received comprehensive intervention guided by ERAS concept.The situation of breastfeeding,psychological status before and after intervention and maternal satisfaction were compared in the two groups.Results In the study group,the onset time of postpartum lactation was shorter than that in the control group,and the lactation amount and breastfeeding rate at 24 hours and 48 hours after the operation were significantly higher than those in the control group,with statistically significant differences(P<0.05).After intervention,the scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)in the study group were lower than those in the control group,and the scores of various satisfaction indexes in the study group were higher than those in the control group,with statistically significant differences(P<0.05).Conclusions The comprehensive intervention guided by ERAS concept can significantly improve the breast-feeding effect of cesarean delivery primipara,alleviate their negative psychological emotions,and improve maternal satisfaction.
