摘要
目的观察非小细胞肺癌(non-small-cell lung cancer,NSCLC)患者中辅助型T细胞22(T helper cell 22,Th22)细胞水平变化及临床意义。方法以250例NSCLC患者为研究对象,同时选取50例健康体检人群为对照组。采用流式细胞仪检测NSCLC组外周血、癌灶组织和癌旁组织以及对照组外周血Th22细胞水平;酶联免疫吸附法检测相关细胞因子白细胞介素-22(interleukin-22,IL-22)和IL-6水平。分析NSCLC患者Th22细胞水平及相关细胞因子水平与临床病理类型的关系,评价Th22细胞水平对NSCLC患者临床分期严重程度的预测价值。结果与对照组比较,NSCLC组患者外周血Th22细胞比例、IL-22水平和IL-6水平均升高(均P<0.05);NSCLC组癌灶组织Th22细胞比例、IL-22水平和IL-6水平均高于外周血和癌旁组织(均P<0.05),而外周血与癌旁组织间上述指标水平比较,差异均无统计学意义(均P>0.05)。NSCLC组外周血、癌旁组织及癌灶组织中Th22细胞比例、IL-22水平及IL-6水平在不同病理类型间比较,差异均无统计学意义(均P>0.05)。NSCLC组患者外周血Th22细胞比例与IL-22(r=0.815,P<0.01)和IL-6(r=0.662,P=0.012)水平均呈正相关;癌灶组织Th22细胞比例也与IL-22(r=0.864,P<0.01)和IL-6(r=0.710,P=0.008)水平均呈正相关;而癌旁组织Th22细胞比例与IL-22(r=0.168,P=0.211)和IL-6(r=0.104,P=0.308)水平均无相关性。受试者工作特征(receiver operating characteristic,ROC)曲线分析显示,癌灶组织Th22细胞比例曲线下面积为0.864(95%CI:0.776~0.953),其最佳工作点为15.2%,此时预测NSCLC患者临床分期严重程度的敏感度和特异度分别为79.5%和83.1%,高于外周血及癌旁组织。结论Th22细胞比例在NSCLC患者外周血、癌旁组织及癌灶组织均较高。其与NSCLC患者的临床分期严重程度密切相关,对于临床分期严重程度的预测,具有一定的应用价值。
Objective To analyse the T-helper 22(Th22)cell level in patients with non-small-cell lung cancer(NSCLC)and its clinical significance.Methods A total of 250 NSCLC patients(NSCLC group)were enrolled in the study.Fifty healthy subjects were served as the control group.The levels of Th22 cells in peripheral blood,cancerous tissues and paracancerous tissues of the NSCLC group,and the levels of Th22 cells in peripheral blood of the control group were detected by flow cytometry.The levels of related cytokines[interleukin-22(IL-22)and IL-6]were detected by enzyme-linked immunosorbent assay(ELISA).The relationship between Th22 cell level and clinicopathological types and related cytokines in NSCLC patients were analyzed.The predictive value of Th22 cell level was evaluated in the clinical stage severity of NSCLC patients.Results Compared with the control group,the levels of Th22 cells and cytokine IL-22 and IL-6 in peripheral blood in NSCLC patients were significantly increased(all P<0.05);The levels of Th22 cells,IL-22 and IL-6 in cancerous tissues were significantly higher than those in peripheral blood and paracancerous tissues of the NSCLC group(all P<0.05),but there was no significant difference between those in peripheral blood and paracancerous tissues(all P>0.05).There was no significant difference in the levels of Th22 cells,IL-22 and IL-6 between peripheral blood,paracanerous tissues and cancerous tissues in different pathological types of the NSCLC group(all P>0.05).Correlation analysis showed that the proportion of peripheral blood Th22 cells in the NSCLC group was positively correlated with IL-22(r=0.815,P<0.01)and IL-6(r=0.662,P=0.012)levels;The proportion of Th22 cells in the tumor tissue of the NSCLC group was also positively correlated with IL-22(r=0.864,P<0.01)and IL-6(r=0.710,P=0.008)levels.In the NSCLC group,there was no significant correlation between the proportion of Th22 cells in adjacent tissues and IL-22(r=0.168,P=0.211)and IL-6(r=0.104,P=0.308)levels.The receiver operating characteristic(ROC)curve analysis showed that the area under curve of Th22 cells in cancerous tissues was 0.864(95%CI:0.776~0.953),and optimal operating point(OOP)was 15.2%,which had 79.5%sensitivity and 83.1%specificity in predicting the severity of clinical stage of NSCLC patients,which was higher than these in peripheral blood and paracanerous tissues.Conclusions Th22 cells are highly expressed in peripheral blood,paracanerous tissues and cancerous tissues of NSCLC patients,which is closely related to the severity of clinical stage of NSCLC patients,and has certain application value for predicting the severity of clinical stage.
