数字化导板引导无牙颌种植手术的效果观察 被引量:7

Effect of computer-aided design in implant restorations for edentulous patients

摘要 目的观察计算机辅助设计数字化导板引导在无牙颌患者种植手术中的应用效果。方法选取2015年7月至2019年10月在温州市人民医院口腔医学中心行人工牙种植修复手术的无牙颌患者35例,术前均行口腔锥形束CT扫描及相应软件术前种植设计。其中应用计算机辅助设计数字化导板引导种植手术20例(A组),植入60枚种植体;自由手种植修复15例(B组),植入40枚种植体。患者术后均行口腔锥形束CT扫描,比较两组患者虚拟种植体和实际种植体的偏差。结果A组患者虚拟种植体和实际种植体肩部偏差为(1.34±0.42)mm,根尖部偏差为(1.29±0.40)mm,角度偏差为(4.56±0.75)°,深度偏差为(0.67±0.24)mm;B组分别为(2.37±0.21)mm、(3.01±0.64)mm、(8.43±2.88)°、(0.80±0.25)mm。A组患者虚拟种植体和实际种植体的颈部、根部、角度偏差均小于B组(均P<0.05),深度偏差与B组比较差异无统计学意义(P>0.05)。结论计算机辅助设计数字化导板引导无牙颌患者种植手术可获得较好的临床效果,其植入准确性可靠,比自由手植入更便利、微创、精准。 Objective To assess the clinical efficacy of implants placed in edentulous jaw using computer-aided design/computer-aided manufacturing surgical guides.Methods A total of 35 patients with dentition defects who underwent tooth implantation and restoration at the Oral Medical Center of Wenzhou People's Hospital were included in our study.Preoperative cone beam computed tomography(CBCT)was performed and preoperative planning was designed with the corresponding software for all 35 patients.Sixty implants of 20 patients were placed based on the assistance of surgical templates(group A),and 40 implants of 15 patients were inserted based on the preoperative planning with free-hand operation(group B).Postoperative CBCT was performed for all 35 patients.Deviations of the implant were measured and compared between the actual and virtual positions.Results There was no significant structure damage during the surgery.A total of 100 implants were successfully inserted.Variations at the implant shoulder,apex and angulation in group A(1.34±0.42 mm,1.29±0.40 mm and 4.56°±0.75°)were significantly lower than those in group B(2.37±0.21 mm,3.01±0.64 mm,8.43°±2.88°,P<0.05),while there was no statistically significant difference in variation of the depth between two groups(P>0.05).Conclusion The use of surgical guide templates can achieve high precision and accuracy in implant shoulder,apex,and angulation,which is much more suitable for complicated procedures.
