摘要
目的:探讨三维数字化重建技术用于鼓室成形精准治疗的临床效果。方法:选择2018年6月-2019年11月中山大学孙逸仙纪念医院和东莞市横沥医院进行鼓室成形术治疗的60例患者作为研究对象,术前随机分组,对照组30例和观察组30例,对照组是按照传统办法行鼓室成形术,观察组是利用三维数字化重建技术用于鼓室成形精准治疗。比较治疗后两组患者外耳道功能、术后不良反应发生情况以及听骨链的CT测量三块听小骨的位置形态。结果:观察组患者听力改善率、术后干耳率高于对照组,胆脂瘤复发率低于对照组,术后干耳平均时间少于对照组(P<0.05);观察组鼓膜穿孔、内耳损伤、鼓室粘连、中耳感染等并发症发生率显著低于对照组(P<0.05)。听骨链的CT测量三块听小骨的位置形态清晰。结论:通过三维数字化重建技术制作听小骨修复体,可以准确修复缺失的解剖结构,形成有效的听骨链结构成分,最大限度地恢复患者的听力,效果较好,且安全性较高,值得推广应用。
Objective:to investigate the clinical effect of 3 d digital reconstruction technology in accurate treatment of tympanoplasty.Methods:in June 2018-November 2019 sun yat-sen memorial hospital, sun yat-sen university and dongguan horizontal drain hospital for tympanoplasty treatment of 60 patients as the research object, preoperative randomized, 30 cases in the control group and 30 cases in the observation group, 30 cases in the control group is in accordance with the traditional method tympanoplasty, observation group is the use of 3 d digital reconstruction technology for the tympanic cavity forming accurate treatment. After treatment, the external auditory canal function, the incidence of postoperative adverse reactions and the position and morphology of the three small auditory bones were compared with that of the two groups.Results:the hearing improvement rate and postoperative dry ear rate of the observation group were higher than the control group, the recurrence rate of cholesteatoma was lower than the control group, and the average postoperative dry ear time was less than the control group(P<0.05). The incidence of complications such as tympanic membrane perforation, inner ear injury, tympanic adhesion and middle ear infection in the observation group was significantly lower than that in the control group(P<0.05). The position and shape of three small auditory bones were measured by CT of ossicular chain.Conclusion:the 3 d digital reconstruction technique can accurately repair the missing anatomical structure, form effective ossicular chain structural components, and maximize the recovery of patients’ hearing.
