摘要
目的:探讨PDCA循环管理在中药饮片处方调剂质量管理中的应用。方法:随机选取医院所有科室中药处方2680张,比较实施PDCA循环管理法前后的差错率,并分析其产生原因并提出干预措施。结果:在2680张处方中共发现8处问题:处方无脚注、处方书写不规范、医师未签字、配伍禁忌、组方结构不清楚、漏抓或误抓药物、剂量超常和发药环节出错。PDCA循环管理法实施前差错处方75张,差错率为5.60%;实施后差错处方15张,差错率为1.12%(P<0.05)。结论:PDCA循环管理法应用于中药饮片处方调剂质量管理中,可及时发现中药处方中存在的问题,降低配药中的差错率,提高了中药饮片的调配质量,同时加强了医务工作者的业务培训和用药安全意识。
Objective:To explore the application of PDCA cycle management in the quality management of prescription dispensation of Chinese herbal decoction pieces.Methods:A total of 2680 prescriptions of Chinese medicine prescribed by our hospital were randomly selected and the error rates were compared before and after implementation of the PDCA cycle management,its causes were analyzed and intervention measures were proposed.Results:A total of 8 kinds of problems were found in the 2680 prescriptions,including no footnotes to prescriptions,irregular writing of prescriptions,no physician’s signature,incompatibility,unclear prescription structure,missed or mistakenly caught drugs,abnormal dosages and errors in the drug delivery process.There were 75 error prescriptions before implementation of PDCA cycle management and only 15 after(5.60%vs 1.12%,P<0.05).Conclusion:Application of PDCA cycle management in the quality management of prescription preparations of Chinese herbal decoction pieces can promptly find problems in the prescription of Chinese medicine,reduce the error rate in dispensation,improve the dispensing quality of Chinese medicine decoction,and strengthen the awareness of the medical training and the medication safety of medical workers.
