摘要
目的研讨自身免疫性肝病相关自身免疫性肝病抗体谱的检测及临床意义。方法选择2018年3月~2020年1月本院收治的自愿参与本组课题研究的自身免疫性肝炎(AIH)患者、原发性胆汁性肝病(PBC)患者、原发性硬化胆管病(PSC)患者及健康受检者各30例,采用间接免疫荧光法测定四组受检者的血清抗核抗体(ANA)、抗线粒体抗体(AMA)、抗平滑肌抗体(SMA)抗体,采用免疫印迹法测定其抗可溶性肝抗原/肝胰抗原(SLA/LP)、抗肝肾微粒体抗体Ⅰ型(LKM-Ⅰ)抗体水平,并比较检测结果。结果AIH组、PBC组、PSC组的血清ANA阳性率高于健康受检组(P<0.05),AIH组的血清ANA阳性率高于PBC组、PSC组(P<0.05)。PBC组的血清AMA阳性率高于AIH组、PSC组、健康受检组(P<0.05)。AIH组的SMA滴度≥1/320、SLA/LP、LKM-Ⅰ阳性率高于PBC组、PSC组、健康受检组(P<0.05)。结论自身免疫性肝病抗体谱用于自身免疫性肝病的临床诊断可行,具有重要的指导价值。
Objective To investigate the detection and clinical significance of antibody spectrum of autoimmune liver disease related to autoimmune liver disease.Methods From March 2018 to January 2020,30 patients with autoimmune hepatitis(AIH),30 patients with primary biliary cirrhosis(PBC),30 patients with primary sclerosing cholangiopathy(PSC)and 30 healthy subjects who voluntarily participated in this project in our hospital were selected.The serum anti nuclear antibody(ANA),anti mitochondrial antibody(AMA)and anti smooth muscle antibody(SMA)antibodies of the four groups of subjects were measured by indirect immunofluorescence.The levels of anti soluble liver antigen/liver hepatopancreas antigen(SLA/LP)and liver kidney microsomal antibody typeⅠ(LKM-Ⅰ)were determined by immunoblotting and the results were compared.Results The positive rates of serum ANA in the AIH group,the PBC group,and the PSC group were higher than those of the healthy group(P<0.05).The positive rate of serum ANA in the AIH group was higher than that of the PBC group and the PSC group(P<0.05).The positive rate of serum AMA in the PBC group was higher than that in the AIH group,the PSC group and the healthy group(P<0.05).The positive rates of SMA titer≥1/320,SLA/LP,and LKM-Ⅰin the AIH group were higher than those in the PBC group,the PSC group and the healthy group(P<0.05).Conclusion The antibody spectrum of autoimmune liver disease is feasible for clinical diagnosis of autoimmune liver disease and has important guiding value.
