摘要
目的探讨孕期妇女妊娠期高血压对早产、低出生体重的影响。方法选取国家妇幼保健中心开展的全国性孕产妇及新生儿健康监测项目所监测的2013年10月至2015年9月期间所有单胎活产孕妇,排除小于18岁、分娩孕周超过42周和重要指标缺失的孕妇后,共有32191例孕妇纳入分析。采用多因素Logistics回归、广义线性回归和多水平模型分析妊娠期高血压与早产、低出生体重的关系。结果妊娠期高血压能显著增加早产(OR=1.63,95%CI:1.26~2.10)和低出生体重(OR=2.91,95%CI:2.29~3.71)的发生风险;孕晚期出现妊娠期高血压的孕妇早产、低出生体重发生风险都明显增加;孕检检出高血压的次数越多,发生早产、低出生体重的风险越高。结论妊娠期高血压会增加早产、低出生体重的发生风险,应该在孕期保健工作中加强对血压的管理和控制,减少妊娠期高血压的发生。
Objective To investigate the effect of gestational hypertension on premature delivery and low birth weight in pregnant women.MethodsData for analysis was from Maternal and Neonatal Health Monitoring Project by Chinese National Center for Maternal and Children′s Health.A total of 32,191 pregnant women with single live birth were included in the analysis after excluding women younger than 18 years old,with more than 42 weeks of gestation and with missing important indicators.Multivariate logistic regression,generalized linear regression and multilevel model were used to analyze the relationship between gestational hypertension with preterm birth and low birth weight.ResultsGestational hypertension can significantly increase the risk of preterm birth(OR=1.63,95%CI:1.26-2.10)and low birth weight(OR=2.91,95%CI:2.29-3.71).The risk of preterm birth and low birth weight was significantly increased in women with gestational hypertension in the third trimester.The more times pregnancy test detects high blood pressure,the higher risk of premature birth and low birth weight.ConclusionGestational hypertension will increase the risk of preterm birth and low birth weight,therefore,it is necessary to strengthen the management and control of blood pressure during pregnancy and reduce the occurrence of gestational hypertension.
