摘要
目的比较高龄初产妇与高龄经产妇产后母乳喂养依从性及产后抑郁发生状况。方法回顾分析2019年8月至2020年3月郑州市第七人民医院60例高龄初产妇与60例高龄经产妇的临床资料,评估并比较两组产后母乳喂养依从性及抑郁发生情况,分析产后母乳喂养依从性与抑郁的关系。结果高龄初产妇母乳喂养依从性低于高龄经产妇,且产后抑郁发生率高于高龄经产妇,差异有统计学意义(P<0.05);高龄初产妇或高龄经产妇依从者抑郁发生率均低于不依从者,差异有统计学意义(P<0.05);回归分析结果显示,产后抑郁是高龄初产妇与经产妇产后母乳喂养依从性不佳的影响因素(OR>1,P<0.05);将全部发生抑郁的产妇作为整体,比较不同抑郁程度产妇的母乳喂养依从性,发现随着抑郁程度加重,母乳喂养依从性越差抑(P<0.05);经等级资料Spearman相关性分析结果显示,产后母乳喂养依从性与产后抑郁程度呈正相关(r>0,P<0.05)。结论高龄初产妇较高龄经产妇母乳喂养依从性更差,可能与产妇产后抑郁有关,且随着抑郁程度加重母乳喂养依从性越差。临床应重视对高龄初产妇及高龄经产妇产后心理干预,尤其是高龄初产妇,应重视产妇产后干预,减少抑郁发生,对提高母乳喂养依从性有积极意义。
Objective To compare the compliance of postpartum breastfeeding and postpartum depression in elderly primiparas and eldeny multiparas.Methods The clinical data of 60 elderly primiparas and 60 advanced multiparas who were treated in Zhengzhou Seventh People’s Hospital from August 2019 to March 2020 were retrospectively analyzed,the postpartum breastfeeding compliance and depression were assessed and compared,and the relationship between postpartum breastfeeding compliance and depression were analyzed.Results The breastfeeding compliance of elderly primiparas was lower than that of elderly multiparas,and the incidence of postpartum depression was higher than that of elderly multiparas,the differences were statistically significant(P<0.05);Both the elderly primipara or the elderly multiparas,the incidence of depression in the compliance was lower than that in the non-compliance(P<0.05);the results of regression analysis showed that postpartum depression was the influencing factor of poor compliance of postpartum breastfeeding in elderly primiparas and elderly multiparas(OR>1,P<0.05);all pregnant women with depression were taken as a whole to compare the breastfeeding compliance of pregnant women with different degrees of depression,it was found that with the aggravation of depression,breastfeeding compliance became worse(P<0.05);according to the Spearman correlation analysis of grade data,postpartum breastfeeding compliance was positively correlated with the degree of postpartum depression(r>0,P<0.05).Conclusions The compliance of breastfeeding in elderly primipara is worse than that in elderly multipara,which may be related to postpartum depression,and with the aggravation of depression,the compliance of breastfeeding was worse.We should pay attention to the psychological intervention for the elderly primipara and the postpartum women,especially for the elderly primipara.We should pay attention to the postpartum intervention to reduce the incidence of depression,which is of positive significance to improve breast-feeding compliance.
