摘要
目的比较单孔与三孔全胸腔镜入路手术治疗早期非小细胞肺癌的有效性及安全性。方法抽取2012年1月至2019年1月开封市人民医院收治的60例早期非小细胞肺癌患者为研究对象,按照随机数字表将其分为单孔组(30例)与三孔组(30例)。单孔组采用单孔全胸腔镜入路手术,三孔组采用三孔全胸腔镜入路手术;观察比较两组的手术相关指标、术后视觉模拟量表(VAS)评分、术后生活质量及并发症发生情况。结果单孔组的手术时间、术后引流时间、术后住院时间均短于三孔组(P<0.05);术中出血量、清扫淋巴结数量、术后引流量均明显少于三孔组(P<0.05)。单孔组术后1、3、5 d的VAS评分均低于三孔组(P<0.05)。单孔组术后生理、心理、躯体、活力评分均明显高于三孔组(P<0.05)。单孔组的并发症发生率为10.00%,低于三孔组的26.67%(P<0.05)。结论相比于三孔全胸腔镜入路,单孔全胸腔镜入路疗效更优,患者痛苦小,生活质量更好,且安全性高。
Objective To compare the efficacy and safety of single-hole and three-hole total thoracic approach in the treatment of early non-small cell lung cancer.Methods Sixty patients with early-stage non-small cell lung cancer admitted in Kaifeng People’s Hospital from January 2012 to January 2019 were selected as the research subjects,and they were randomly divided into single hole group(30 cases)and three hole group(30 cases)according to random digital table.The single-hole group underwent single-hole thoracoscopic approach surgery,and the three-hole group underwent three-hole total thoracoscopic surgery;the operation-related indicators,postoperative visual analogue scale(VAS)scores,postoperative quality of life and complications of the two groups were observed and compared.Results The operation time,postoperative drainage time,and postoperative hospital stay in the single-hole group were significantly shorter than those in the three-hole group(P<0.05);intraoperative blood loss,number of lymph nodes cleaned,and postoperative drainage volume were significantly less than those in three-hole group(P<0.05).The VAS scores at 1 day,3 days,and 5 days after operation in the single-hole group were significantly lower than those in the three-hole group(P<0.05).The postoperative physiological,psychological,physical,and vitality scores in the single-hole group were significantly higher than those in the three-hole group(P<0.05).The incidence rate of complications in the single-hole group was 10.00%,which was significantly lower than that in the three-hole group(26.67%),P<0.05.Conclusions Compared with the three-hole thoracoscopic approach,the single-hole thoracoscopic approach is more effective,the patient has less pain,better quality of life,and high safety.
