摘要
目的:探讨原因不明复发性流产(URSA)及先兆流产患者外周血单核细胞上T细胞免疫球蛋白黏蛋白-3(Tim-3)、程序性死亡因子-1(PD-1)的表达特征及临床意义。方法:选取URSA、先兆流产早孕妇女和正常早孕妇女各80例。采集外周血分离单个核细胞,检测单核细胞上Tim-3、PD-1表达水平,并测定外周血IL-4、IL-12、IFN-γ水平。结果:Tim-3、PD-1、 IL-4水平URSA组最低,先兆流产组次之,正常组最高;IL-12和IFN-γ水平URSA组最高,先兆流产组次之,正常组最低(均P<0.05)。相关分析显示IL-12与IL-4、Tim-3、PD-1水平均呈负相关,与IFN-γ呈正相关(P均<0.05);IL-4与IFN-γ、Tim-3、PD-1均呈正相关(P均<0.05);IFN-γ与Tim-3、PD-1均呈负相关(P均<0.05);Tim-3与PD-1呈正相关(P<0.05)。结论:URSA及先兆流产的发生可能与患者外周血单核细胞上Tim-3、PD-1表达水平降低有关,临床可将其作为治疗URSA和先兆流产的潜在靶点。
Objective: To investigate the expression characteristics and clinical significance of T-cell immunoglobulin-3(Tim-3) and programmed death factor-1(PD-1) in peripheral blood monocytes of women with unexplained recurrent abortion(URSA) or threatened abortion. Methods: 80 women with URSA were selected in group A, women with threatened abortion during the first trimester of pregnancy were selected in group B, and 80 normal pregnant women during the first trimester of pregnancy were selected in group C. Peripheral blood of these women in the three groups were collected and the mononuclear cells were isolated. The expression levels of Tim-3 and PD-1 in monocytes of women were detected, and serum IL-4, IL-12, IFN-γlevels of these women were measured. Results: The Tim-3 level, PD-1 level, or IL-4 level of women in group A was the lowest, and which of women in group C was the highest. The IL-12 level or IFN-γlevel of women in group A was the highest, and which of women in group C was the lowest(all P<0.05). Correlation analysis showed that IL-12 level was negatively correlated with IL-4 level, Tim-3 level, or PD-1 level, but was positively correlated with IFN-γlevel(all P<0.05). IL-4 level was positively correlated with IFN-γlevel, Tim-3 level, or PD-1 level, but IFN-γlevel was negatively correlated with Tim-3 level or PD-1 level(all P<0.05). Tim-3 level was positively correlated with PD-1 level(P<0.05). Conclusion: The occurrence of URSA and threatened abortion may be related to the decrease of the expression levels of Tim-3 and PD-1 of monocytes in peripheral blood of women, which can be used as a potential target spot for treating URSA and threatened abortion.
