320排CT在肺占位性病变诊断中的应用价值 被引量:5

Clinical Application Value of 320-slice CT in the Diagnosis of Pulmonary Space-occupying Lesions

摘要 目的探讨320排计算机断层扫描(computed tomography,CT)血管成像及三维重建技术在肺占位性病变中的诊断价值。方法回顾性分析2017-01/2018-12月在作者医院行320排CT检查且行外科手术治疗的63例肺占位性病变患者病历资料。与手术后病理结果对照评价320排CT血管成像及三维重建技术对肺占位性病变的诊断价值。结果所有患者均一次性成功完成320排CT靶扫描检查,均获得较高质量的肺占位性病变三维重建图像。63例患者经术后病理诊断与320排CT靶扫描、图像处理后诊断考虑恶性病变47例,良性病变16例;其中320排CT考虑恶性术后病理诊断亦为恶性44例,320排CT考虑恶性术后病理诊断为良性3例;320排CT考虑良性术后病理诊断亦为良性13例,320排CT考虑良性术后病理诊断为恶性3例。诊断良恶性倾向符合率为90.5%,320排CT与病理诊断结果差异无统计学意义(P>0.05)。所有患者320排CT血管成像及三维重建技术成像均获得了满意的气管、肺动脉、肺静脉、肺占位性病变三维关系,清楚显示心脏大血管形态及解剖变异情况。结论320排CT血管成像及三维重建技术能清晰显示肺占位性病变及其与周围重要器官、组织的关系,可作为肺占位病变患者手术前检查的常规手段。 Objective To explore the clinical application value of 320-slice computed tomography(CT)angiography and three-dimensional reconstruction technology in the diagnosis of pulmonary space-occupying lesions.Methods A total of 63patients with pulmonary space-occupying lesions who underwent 320-slice CT examination and surgical treatment at author's hospital from January 2017to December 2018were retrospectively analyzed.The value of 320-slice CT angiography and three-dimensional reconstruction technology in patients with pulmonary space-occupying lesions were evaluated by comparing with the postoperative pathological results.Results The 320-slice CT scans were successfully completed at one time and all obtained excellent image quality of all patients.Among the 63patients,including 47patients were malignant lesions,16cases were benign lesions.Among them,44patients were diagnosed as malignant by postoperative pathological and 320-slice CT results,3patients were diagnosed as malignant by 320-slice CT results while postoperative pathological results showed benign lesions;13patients were diagnosed as benign by postoperative pathological and 320-slice CT results,3patients were diagnosed as malignant by postoperative pathological results but not by 320-slice CT results.The diagnostic coincidence rate was 90.5%,and there was no statistically significant difference between 320-slice CT results and pathology diagnosis(P>0.05).The satisfactory of three-dimensional relationship of trachea,pulmonary artery,pulmonary vein and pulmonary space-occupying lesions were obtained of all patients with 320-slice CT angiography and threedimensional reconstruction imaging,which clearly showed the morphology and anatomical variation of the heart and large vessels.Conclusion The 320-slice CT angiography and three-dimensional reconstruction technology can clearly show the relationship between pulmonary space-occupying lesions and their adjacent important organs and tissues,which can be used as routine methods for preoperative examination of pulmonary space-occupying lesions.
