摘要
目的探讨糖尿病运动处方对新诊断2型糖尿病(nT2DM)患者的握力、糖脂代谢及胰岛素抵抗(IR)的影响。方法选取2018年8月-2019年7月南京市溧水区人民医院收治的nT2DM患者90例,随机分为2组,每组各45例,对照组行有氧运动训练,观察组采用糖尿病运动处方训练方案(有氧+弹力带抗阻),2组均持续训练12周,记录并比较干预前后2组患者握力、糖脂代谢相关指标、IR程度的变化。结果①干预12周后,两组FPG、HbAlc、TG、TC、LDL-C、Fins、HOMA-IR均低于干预前,观察组握力高于干预前(P<0.05);②干预12周后,观察组FPG、HbAlc、TG、TC、LDL-C低于对照组,握力高于对照组(P<0.05);③两组不良反应发生率比较,差异无统计学意义(χ^2=0,P>0.05)。结论糖尿病运动处方与有氧运动均可以改善nT2DM患者的糖脂代谢及IR,但在增强患者握力方面,糖尿病运动处方较单纯有氧运动更佳。
Objective To explore the effect of diabetes exercise prescription on grip strength,glucose and lipid metabolism and insulin resistance(IR)in newly diagnosed type 2 diabetic mellitus(nT2DM).Methods 90 newly diagnosed type 2 diabetic mellitus admitted Nanjing Lishui People′s Hospital from August 2018 to July 2019 were randomly divided into two groups.45 cases in the control group were trained for aerobic exercise and 45 cases in the observation group adopt diabetic exercise prescription training program(aerobic+elastic band resistance).Both groups continued to be trained for 12 weeks.The changes of grip strength,glucose and lipid metabolism related indexes and IR degree of the two groups of patients before and after intervention were recorded and compared.Results①After 12 weeks of intervention,FPG,HbAlc,TG,TC,LDL-C,Fins,and HOMA-IR of the observation group and the control group were significantly lower than before the intervention.The grip strength of the observation group was significantly higher than before,the difference was statistically significant(P<0.05);②After 12 weeks of intervention,compared with the control group after intervention,FPG、HbAlc、TG、TC、LDL-C of the observation group was lower and the grip strength was higher;③The difference was statistically significant(χ^2=0,P<0.05).Conclusion Both diabetes exercise prescription and aerobic exercise can improve glucose and lipid metabolism,insulin resistance in newly diagnosed type 2 diabetic mellitus,but in terms of enhancing the grip strength of patients,diabetes exercise prescription is better than simple aerobic exercise.
