摘要
目的观察针刀联合身痛逐瘀汤加减方治疗瘀血阻络型大偻的临床疗效。方法随机将60例瘀血阻络型大偻患者分为针刀组、身痛逐瘀汤加减方组和治疗组,每组各20例。身痛逐瘀汤加减方组予以身痛逐瘀汤加减方口服治疗,针刀组予以针刀松解治疗,治疗组予以身痛逐瘀汤加减方联合针刀松解治疗,3组均以2个月为治疗疗程。结果治疗前3组患者腰骶部疼痛程度(VAS评分)、晨僵时间、血沉(ESR)、C反应蛋白(CRP)、胸廓活动度、脊柱活动度、强直性脊柱炎功能指数(BASFI)、Bath强直性脊柱炎疾病活动指数(BASDAI)、Schober试验水平均相当,P>0.05,差异无统计学意义;治疗后3组患者上述指标、评分均改善,治疗组患者各项评分及实验室指标明显优于针刀组与中药组,P<0.05,差异具有显著的统计学意义。结论针刀联合身痛逐瘀汤加减方疗法是对瘀血阻络型强直性脊柱炎患者具有显著疗效的一种治疗方法。
Objective To observe the clinical efficacy of needle knife combined with Shentong Zhuyu Decoction addition and subtraction in the treatment of blood stasis type ankylosing spondylitis(AS).Methods Sixty patients with AS were randomly assigned to needle knife group,Shentong Zhuyu Decoction group and treatment group,20 cases in each group.The Shentong Zhuyu Decoction group was treated with Shentong Zhuyu Decoction addition and subtraction,the needle knife group was treated with small needle knife,and the treatment group was treated with Shentong Zhuyu Decoction addition and subtraction and small needle knife.All groups were treated for 2 months.Results The pain of lumbosacral(VAS score),morning stiffness time,ESR,CRP,and the thoracic activity,spinal activity,BASFI,BASDAI and Schober test of the three groups before treatment were similar,P>0.05,the difference was not statistically significant.After treatment,the above indexes and scores of the three groups were improved.The scores and laboratory indexes of patients in the treatment group were significantly better than those in the needle-knife group and the Shentong Zhuyu Decoction group,P<0.05,and the difference was statistically significant.Conclusion The treatment of needle knife combined with Shentong Zhuyu Decoction addition and subtraction is a kind of treatment for patients with stasis and obstructive AS.
