摘要
目的探讨McMaster模式家庭治疗在结直肠癌院外护理中的应用效果。方法选取我院2017年3月至2018年3月收治的98例结直肠癌患者为研究对象,以随机数字表法分为对照组(n=49,实施常规干预)与观察组(n=49,在对照组基础上实施McMaster模式家庭治疗干预)。比较两组干预前后症状自评量表(SCL90)、健康行为自我评价量表(SRAHP)、家庭功能评定量表(FAD)评分。结果干预前,两组各项SCL90评分(躯体化、强迫症状、人际关系敏感、抑郁、焦虑、敌对、恐怖、偏执、精神病性)、各项SRAHP评分(运动、健康责任、营养、心理安适评分及总分)和各项FAD评分(问题解决、沟通、角色、情感反应、情感介入、行为控制、总的功能)比较差异均无统计学意义(均P>0.05)。干预后,两组各项SCL90评分和FAD评分均低于干预前,且观察组的各项SCL90评分和FAD评分均低于对照组,差异有统计学意义(均P<0.05);两组各项SRAHP评分均高于干预前,且观察组的各项SRAHP评分均高于对照组,差异有统计学意义(均P<0.05)。结论在结直肠癌患者的院外护理干预中实施McMaster模式家庭治疗能够改善患者心理状态,提升患者的自我效能及改善家庭相关功能。
Objectives To investigate the effect of McMaster family therapy in nursing care for patients with colorectal cancer out of hospital.Methods Ninety-eight patients with colorectal cancer treated at our hospital between March 2017 and March 2018 were recruited and randomly assigned to the treatment group(n=49)and control group(n=49).Patients in the control group received routine care out of hospital,and those in the treatment group received,in addition to routine care,the McMaster family therapy.The symptom checklist 90(SCL-90),the self-rated abilities for health practices scale(SRAHP),and family assessment device(FAD)were compared between the two groups before and after the intervention.Results Scores of SCL-90(scores of so⁃matization,obsessive-compulsive symptoms,interpersonal sensitivities,depression,anxiety,hostility,terror,paranoia,and psychot⁃ic),SRAHP(scores of exercise,health responsibility,nutrition,psychological safety,and total scores),and FAD(scores of prob⁃lem-solving,communication,role,emotional response,emotional intervention,behavioral control,and general function)did not dif⁃fer between the two groups before intervention(P>0.05).After the intervention,scores of SCL-90 and FAD decreased,and SRAHP increased in both groups.Scores of SCL-90 and FAD were significantly lower,and scores of SRAHP were significantly higher in the treatment group than in the control group(P<0.05).Conclusion In patients with colorectal cancer,the McMaster family therapy in nursing care out of hospital can improve patients’psychological status,self-efficacy,and family function.
