摘要
目的比较超声与CT在小儿急性阑尾炎术前诊断中的应用价值。方法回顾性分析我院103例术前诊断为急性阑尾炎的4~12岁小儿患者。将术前阑尾超声与腹部CT的结果进行对比,计算超声与CT诊断小儿急性阑尾炎的灵敏度、特异度及准确度,进行统计学分析,比较两组的差异。结果超声诊断小儿急性阑尾炎的灵敏度、特异度、准确度分别为90.6%、71.4%、89.3%;CT诊断的灵敏度、特异度、准确度分别为94.8%、42.9%、91.3%。超声与CT在灵敏度及准确度方面无明显统计学差异(P>0.05),而诊断特异度超声优于CT(P<0.05)。结论超声检查对诊断小儿急性阑尾炎具有较高的准确度,且灵敏度及特异度与腹部CT无明显差异,应作为诊断小儿急性阑尾炎的首选影像学检查方法。
Objective To compare the value of ultrasound and CT in the preoperative diagnosis of acute appendicitis in children.Methods Retrospective analysis was conducted on 103 patients aged 4-12 years who were diagnosed with acute appendicitis before operation in our hospital.The results of preoperative appendicitis ultrasound and abdominal CT were compared to evaluate the sensitivity,specificity and accuracy of ultrasound and CT in the diagnosis of acute appendicitis in children.Statistical analysis was conducted to compare the differences between the two groups.Results The sensitivity,specificity and accuracy of ultrasonic diagnosis of acute appendicitis in children were 90.6%,71.4%and 89.3%,respectively.The sensitivity,specificity and accuracy of CT diagnosis were 94.8%,42.9%and 91.3%,respectively.There was no significant difference in sensitivity and accuracy between ultrasound and CT(P>0.05),while the diagnostic specificity of ultrasound was significantly higher than that of CT(P<0.05).Conclusion Ultrasonic examination has higher accuracy in the diagnosis of acute appendicitis in children,and its sensitivity and specificity are not significantly different from those of abdominal CT.Therefore,it should serve as the preferred imaging examination method for the diagnosis of acute appendicitis in children.
