摘要
目的了解泉州市2016年—2018年胎儿结构畸形的分布特点。方法孕妇定期接受产期检查时超声或MRI检查发现胎儿结构畸形,将胎儿结构畸形按器官系统、患者年龄、生育情况、诊断孕周进行归类。结果2016年1月—2018年12月我院产前诊断中心接诊的5250例患者中,胎儿结构畸形共4159例。其中多器官畸形798例,占19.2%;神经系统畸形519例,占12.5%;心血管系统畸形1062例,占25.5%;泌尿系统畸形516例,占12.4%;消化系统畸形117例,占2.8%;骨骼系统畸形187例,占4.5%;唇腭裂324例,占7.8%;腹腔肿物122例,占2.9%;胸腔肿物135例,占3.2%;颈部肿物16例,占0.3%%;水囊瘤142例,占3.4%;膈疝31例,占0.7%;腹裂和脐膨出55例,占1.3%;足内翻畸形69例,占1.7%;其他结构畸形66例,占1.6%。胎儿结构畸形就诊的孕妇最小年龄15岁,最大46岁,平均年龄(28.7±5.2)岁,结构畸形诊断最小孕周为10+5周,最大42+1周,平均(25±5.7)周,初产妇1384例,占33.3%,经产妇2775例,占66.7%,高龄孕妇202例,占4.9%。结论胎儿结构畸形是出生缺陷的重要组成部分,了解胎儿结构畸形的分布特点对改善其预后具有重要帮助。
Objective To understand the distribution characteristics of fetal structural malformation in Quanzhou from 2016 to 2018 years.Methods Ultrasound or MRI examinationfound fetal structural malformation when pregnant women receive regular prenatal examination,which was classified by the organ system,patient age,fertility status,and diagnostic gestational age.Results Among the 5250 patients received in the prenatal diagnosis center of our hospital in January 2016 to December 2018,fetal structural malformation occurred in 4159 cases,including multi-organ malformation in 798 cases,accounting for 19.2%;nervous system malformation in 519 cases,12.5%;cardiovascular system malformation in 1062 cases,25.5%;urinary system malformation in 516 cases,12.4%;digestive system malformation in 117 cases,2.8%;skeletal system malformation in 187 cases,4.5%;cleft lip and palate in 324 cases,7.8%;abdominal mass in 122 cases,2.9%;thoracic mass in 135 cases,3.2%;neck mass in 16 cases,0.3%;hygroma in 142 cases,3.4%;diaphragmatic hernia in 31 cases,0.7%;gastroschisis and acromphalus in 55 cases,1.3%;eguinovarus foot in 69 cases,1.7%;other structural malformation in 66 cases,1.6%.The minimum age of patients with fetal structural malformation is 15 years old,the maximum age is 46 years old,average age of 28.7±5.2 years old,the minimum gestational age for structural malformation is 10+5 weeks,and the maximum is 42+1 weeks,average of 25 weeks.There were 1384 primipara,accounting for 33.3%,2775 multipara,66.7%,and 202 women in advanced maternal age,4.9%.Conclusion Fetal structural malformation constitutes an important part of birth defects,to understand the distribution characteristics of fetal structural malformation,it is important to improve the prognosis of fetus with structural malformation.
