摘要
目的探讨抗核抗体(ANA)及抗核抗体谱(ANAs)在原发性血小板减少症患者诊断中的应用价值。方法选取在2016年1月至2020年3月期间我院收治的114例初步诊断为原发性血小板减少症的患者作为研究对象。所有患者采用间接免疫荧光方法测定ANA,采用欧盟印迹法测定ANAs。对比分析ANA、ANAs的阳性率。结果所有患者进行检测后发现,ANAs阳性率为44%,明显高于ANA阳性率37%,差异有统计学意义(P<0.05);进行ANAs检测,患者的确诊率为88%,明显高于进行ANA检测患者的确诊率16%,差异有统计学意义(P<0.05);ANAs的抗SSA、抗Ro-52、抗SSB、抗NRNP/SM、抗核糖体P蛋白、抗Sm、抗Scl-70、抗Jo-1中,抗SSA阳性率为80%,抗Ro-52阳性率为58%,抗SSB阳性率为10%、抗NRNP/SM阳性率为56%、抗核糖体P蛋白阳性率为50%、抗Sm阳性率为4%、抗Scl-70阳性率为0、抗Jo-1阳性率为0,差异有统计学意义(P<0.05)。结论ANA及ANAs检测对原发性血小板减少症患者的临床诊断具有重要意义,能及时让临床医生掌握患者患病情况,指导医生采取有效的预防、治疗措施,值得临床推广。
Objective To investigate the application value of ANA and ANAs spectrum in the diagnosis of the patients with primary thrombocytopenia.Methods 114 patients preliminarily diagnosed with thrombocytopenia who were admitted to our hospital from January 2016 to March 2020 were selected as research objects.ANA was determined by indirect immunofluorescence in all the patients,and ANAs by European Blotting.The positive rates of ANA and ANAs were compared and analyzed.Results The test results showed that the positive rate of ANAs was 44%,significantly higher than that of ANA which was 37%,with statistically significant difference(P<0.05).The diagnosis rate of the patients with ANAs test was 88%,significantly higher than that of the patients with ANA test which was 16%,and the difference was statistically significant(P<0.05).The positive rates of anti SSA,anti Ro-52,anti SSB,anti NRNP/SM,anti-ribosomal P protein,anti SM,anti ScL-70 and anti Jo-1 of ANAS were respectively 80%,58%,10%,56%,50%,4%,0 and 0,with statistically significant difference(P<0.05).Conclusion ANA and ANAs tests are of great significance in the clinical preliminary diagnosis of thrombocytopenia.They can timely inform clinicians of the patients’condition and guide them to take effective preventive and therapeutic measures,which is worthy of clinical promotion.
