摘要
为了研究按服务项目支付、按总额支付和混合支付3种支付方式对慢性疾病患者及医院预防努力的激励作用,以患者效用、医院收益及社会总效用最大为目标建立优化模型.分析了混合支付方式中混合比例系数对患者自我预防努力、社区医院预防努力和三级医院治疗努力的影响,计算了3种支付方式和社会总效用最大时患者自我预防努力、社区医院预防努力及三级医院治疗努力的均衡解,比较了3种支付方式的社会总效用值.结果表明,混合支付方式下患者自我预防努力、社区医院预防努力及社会总效用最大.政府采用混合支付方式有助于激励患者和医院采取预防努力,并实现社会总效用最大.
To study the incentive effect of pay by service item,pay by total amount and mixed payment on the preventive efforts of chronic disease patients and hospitals,an optimization model was established for maximizing the patient utility,the hospital revenue and the total social utility.The influences of the mixed proportion coefficient on the self-preventive effort of patients,the preventive effort of community hospital and the treatment effort of tertiary hospital were analyzed.The equilibrium solutions of the self-preventive effort of patients,the preventive effort of community hospital and the treatment effort of tertiary hospital on three payment modes and the maximum of total social utility were calculated.The total social utility values of the three payment modes were compared.The results show that the self-preventive effort of patient,the preventive effort of community hospital,and the total social utility are maximum on the mixed payment mode.The government's use of the mixed payment will help to encourage the patients and the hospitals to take preventive efforts and achieve the maximum total social utility.
