摘要
目的:研究基于免管免禁快速康复外科理念的综合疗法对食管癌患者的康复预后及身心状态的影响。方法:选取2016年6月-2018年12月,于笔者所在医院胸心外科拟行微创手术治疗的食管癌患者94例记为观察对象,通过随机抽签法将其均分成研究组和对照组。对照组予以常规围术期干预,研究组则予以基于免管免禁快速康复外科理念的综合疗法干预。分别比较两组术后首次下床活动时间、排气时间、住院时间、医疗费用,以及术后不同时间点切口疼痛情况、并发症发生情况、30 d死亡率等方面的差异。结果:研究组与对照组术后首次下床活动时间、排气时间、住院时间、医疗费用方面对比,前者更低(均P<0.05)。术后1、10、30 d时,研究组切口疼痛VAS评分均低于对照组(P<0.05)。研究组与对照组在术后并发症发生率方面对比,前者更低(P<0.05)。研究组与对照组在死亡率方面对比差异无统计学意义(P>0.05)。结论:免管免禁快速康复外科理念的综合疗法应用于微创食管癌手术患者的效果显著,有利于促进患者早日康复,改善免疫功能,降低术后并发症发生率,值得临床推广应用。
Objective:To study the influence of comprehensive therapy based on the concept of non-tube no fasting of rapid rehabilitation surgery on the rehabilitation prognosis and physical and mental state of patients with esophageal cancer.Method:From June 2016 to December 2018,94 cases of esophageal cancer patients who were going to receive minimally invasive surgery in the department of cardiothoracic surgery of our hospital were selected as the observation objects,and all of them were divided into the study group and the control group by random lottery digital table method.The control group was given routine perioperative intervention,while the study group was given comprehensive therapy intervention based on the concept of non-tube no fasting of rapid rehabilitation surgery.The differences between the two groups were compared in terms of the first time of getting out of bed after surgery,exhaust time,hospitalization time,and medical expenses,pain of incision at different time points after surgery,postoperative complications,and postoperative mortality 30 days after surgery.Result:Compared the study group and control group,the first time of getting out of bed after surgery,exhaust time,hospitalization time and medical expenses were lower in the former,the differences were statistically significant(P<0.05).The scores of incision pain VAS in the study group were lower than those in the control group at 1 d,10 d and 30 d after surgery,the differences were statistically significant(P<0.05).The incidence of postoperative complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in mortality between the study group and the control group(P>0.05).Conclusion:The comprehensive therapy with the concept of free administration and non-tube no fasting of rapid rehabilitation surgery has a significant effect on minimally invasive esophageal cancer surgery patients,which is beneficial to promote the early recovery of patients,improve the immune function,reduce the incidence of postoperative complications,and is worthy of clinical promotion and application.
