摘要
目的探讨加味桃红四物汤对混合痔外剥内扎术后疼痛的应用价值。方法收集90例2017年2月—2018年2月混合痔外剥内扎术后疼痛患者。按照治疗分组,常规治疗干预组采取高锰酸钾溶液熏洗联合常规灌肠等治疗,患者手术结束后的第2天用甲硝唑葡萄糖注射液50 mL灌肠保留15 min,用1∶5000高锰酸钾融合热水熏蒸,熏蒸30 min。每天均1次,治疗1周。加味桃红四物汤组则采取常规治疗联合加味桃红四物汤治疗。加味桃红四物汤组方:红花15 g,桃仁15 g,川芎15 g,赤芍15 g,地榆15 g,冰片15 g,龙血竭15 g,五倍子15 g,当归15 g。水煎服,每天早晚各1次,治疗1周。比较两组疗效;伤口疼痛持续的时间、应用止痛药物的天数、伤口的平均愈合时间;治疗前后患者伤口疼痛模拟评分、生活质量评分;平均住院的费用。采用SPSS22.0软件对所得数据进行统计分析,计量资料用均数±标准差(—x±s)表示,比较采用t检验;计数资料以率(%)表示,比较采用χ^2检验,P<0.05为差异有统计学意义。结果加味桃红四物汤组疗效高于常规治疗干预组,P<0.05;加味桃红四物汤组伤口疼痛持续的时间、应用止痛药物的天数、伤口的平均愈合时间优于常规治疗干预组,P<0.05;治疗前两组伤口疼痛模拟评分、生活质量评分接近,P>0.05;治疗后加味桃红四物汤组伤口疼痛模拟评分、生活质量评分优于常规治疗干预组,P<0.05。加味桃红四物汤组平均住院的费用显著低于常规治疗干预组,P<0.05。结论常规治疗联合加味桃红四物汤治疗混合痔外剥内扎术后疼痛的疗效好,可有效减轻术后疼痛,减少止痛药物的使用,提高患者的生活质量,减少医疗费用。
Objective To explore the application value of Modified Taohong Siwu Decoction in the treatment of postoperative pain of mixed iliac exfoliation.Methods A total of 90 patients with pain in the hospital from February 2017 to February 2018 were treated with external hemorrhoids.According to the treatment group,the conventional treatment intervention group was treated with potassium permanganate solution and conventional enema.The patient was treated with metronidazole glucose injection 50 mL enema for 15 minutes on the second day after surgery,with 1∶5000 high manganese.Potassium acid is mixed with hot water for fumigation and fumigation for half an hour.Once a day,treat for 1 week.Jiaxing Taohong Siwu Decoction group was treated with conventional treatment combined with flavored Taohong Siwu Decoction.Modified Taohong Siwu Decoction:safflower 15g,peach kernel 15g,Chuanxiong 15 g,red peony 15 g,mantle 15 g,borneol 15 g,dragon blood 15 g,gallnut 15 g,angelica 15 g.decoct in water for oral administratin,once a day in the morning and evening.Treatment for 1 week.The efficacy of the two groups was compared;the duration of wound pain,the number of days of application of analgesic drugs,the average healing time of wounds;the simulated pain score of patients with pain before and after treatment,the quality of life score;the average hospitalization cost.The data were statistically analyzed by SPSS22.0 software.The measurement data were expressed by the mean standard deviation(x—±s).The comparison was performed by t test.The count data was expressed by the rate(%).The comparison was performed byχ^2 test,P<0.05 was the difference.It is statistically significant.Results The effect of Modified Taohong Siwu Decoction group was higher than that of conventional treatment intervention group(P<0.05).The duration of wound pain,the number of days of application of analgesic drugs and the average healing time of wounds in Modified Taohong Siwu Decoction group were better than those of conventional treatment intervention group,P<0.05;before the treatment,the wound pain simulation score and the quality of life score were close to P>0.05.After treatment,the wound pain simulation score and quality of life score were better than the conventional treatment intervention group(P<0.05).The average hospitalization cost of Modified Taohong Siwu Decoction group was significantly lower than that of the conventional treatment intervention group,P<0.05.Conclusion Conventional treatment combined with flavored Modified Taohong Siwu Decoction is effective in treating postoperative pain after sputum exfoliation and internal ligation.It can effectively reduce postoperative pain,reduce the use of analgesic drugs,improve patients'quality of life and reduce medical expenses.
