摘要
目的分析超声造影鉴别诊断肝脏肿瘤的效果,以提高疾病诊断的准确率。方法筛选出本院2019年1月到2020年1月期间诊治的100例肝脏局灶性病变患者作为研究对象,所有患者均经手术病理证实,其中良性病变43例,恶性病变57例;术前对患者行常规超声检查与超声造影检查,以手术病理诊断结果为标准,分析常规超声检查与超声造影检查的诊断结果,并评价两种检查方法对肝脏肿瘤的鉴别诊断效果,计算准确度、敏感度、特异度、阳性预测值、阴性预测值。结果常规超声检查检出良性病灶33例,恶性病灶41例;超声造影检查检出良性病灶40例,恶性病灶55例。常规超声检查诊断肝脏肿瘤的准确度、敏感度、特异度、阳性预测值、阴性预测值依次为74.0%、71.9%、76.7%、80.4%、67.3%;超声造影检查诊断肝脏肿瘤的准确度、敏感度、特异度、阳性预测值、阴性预测值依次为95.0%、96.5%、93.0%、94.8%、95.2%;超声造影检查均高于常规超声检查,P<0.05。结论超声造影鉴别诊断肝脏肿瘤的效果显著,准确度、敏感度、特异度、阳性预测值、阴性预测值均较高。
Objective To analyze the effect of contrast-enhanced ultrasound in differential diagnosis of liver tumors in order to provide reference for clinical diagnosis and improve the accuracy of disease diagnosis.Methods A total of 100 patients with liver focal lesions diagnosed and treated in our hospital from January 2019 to January 2020 were selected as the research objects.All patients were confirmed by operation and pathology,including 43 benign lesions and 57 malignant lesions.Routine ultrasound and contrast-enhanced ultrasound were performed on patients before operation,and the results of routine ultrasound and contrast-enhanced ultrasound were analyzed based on the results of operation and pathology The accuracy,sensitivity,specificity,positive predictive value and negative predictive value were calculated.Results 33 cases of benign lesions and 41 cases of malignant lesions were detected by conventional ultrasound,40 cases of benign lesions and 55 cases of malignant lesions were detected by contrast-enhanced ultrasound.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value were 74.0%,71.9%,76.7%,80.4%and 67.3%respectively;the accuracy,sensitivity,specificity,positive predictive value and negative predictive value of contrast-enhanced ultrasound were 95.0%,96.5%,93.0%,94.8%and 95.2%respectively;the contrast-enhanced ultrasound was higher than that of conventional ultrasound Ultrasonic examination,P<0.05.Conclusion Contrast-enhanced ultrasound has a significant effect in the differential diagnosis of liver tumors with high accuracy,sensitivity,specificity,positive predictive value and negative predictive value.
