摘要
目的探讨抑郁、心理弹性对心内科老年患者自我护理能力的影响及衰弱的调节效果。方法选取2016年1月至2018年6月入住本院的342例心内科老年患者为研究对象,通过调查问卷,采用老年抑郁量表(GDS-30)、心理弹性量表简版(CD-RISC-10)、老年自我护理能力量表(ESCA)、Tilburg衰弱评估量表(TFI)评分,采用Pearson相关分析确定抑郁、心理弹性与自我护理能力的相关性;采用多重线性回归确定心内科老年患者衰弱程度对抑郁、心理弹性及自我管理能力的调节效果。结果GDS-30评分与CD-RISC-10评分、ESCA评分为负相关(P<0.05);GDS-30评分与TFI评分为正相关(P<0.05);CD-RISC-10评分与ESCA评分呈正相关(P<0.05);CD-RISC-10评分与TFI评分呈负相关(P<0.05);ESCA评分与TFI评分呈负相关(P<0.05);抑郁在自护能力及衰弱程度间具有负向调节效应,心理弹性在自护能力及衰弱程度间具有正向调节效应。结论老年心内科抑郁、心理弹性均对自护能力及衰弱具有调节效应,应注重对心内科老年患者抑郁情绪、心理弹性下降的监测,强化心理护理,减轻老年患者抑郁程度,有助于提升其自我护理能力,降低衰弱程度。
Objective To explore the influence and effect of debilitating adjustment of depression and psychological resilience on the self-care ability of elderly patients in cardiology.Methods To select 342 cases of elderly patients in cardiology who were admitted to our hospital from January 2016 to June 2018 as the study objects,the old geriatric depression scale(GDS-30),the 10-item Connor-Davidson resilience scale(CD-RISC-10),elderly exercise of self-care agency scale(ESCA)and Tilburg frailty indicator(TFI)scores were adopted through the questionnaire,Pearson correlation analysis was used to determine the correlation between depression,psychological resilience and self-care ability,and multiple linear regression was used to determine the adjustment effect of the debilitating degree of elderly patients in cardiology on depression,psychological resilience and self-management ability.Results The GDS-30 score was negatively correlated with CD-RISC-10 and ESCA scores(P<0.05),the GDS-30 score was positively correlated with TFI score(P<0.05),the CD-RISC-10 score was positively correlated with ESCA score(P<0.05),the CD-RISC-10 score was negatively correlated with TFI score(P<0.05),and ESCA score was negatively correlated with TFI score(P<0.05).The depression had a negative adjustment effect between self-protection ability and the debilitating degree,and psychological resilience had a positive adjustment effect between self-care ability and the debilitating degree.Conclusion The depression and psychological resilience in elderly patients in cardiology had a regulatory effect on self-care ability and debilitating degree,medical staff should pay attention to monitor the decline of depression and psychological resilience in elderly patients in cardiology,and intensive psychological nursing to alleviate the depression of elderly patients,which was help to improve patients'self-care ability and reduce the debilitating degree.
