摘要
目的分析Castleman病的多层螺旋CT(MSCT)表现,总结病变的影像学特征,寻找有助于提高CT诊断水平的征象。方法回顾性收集2010年6月至2018年7月于云南省昭通市第一人民医院行MSCT检查并经术后病理证实的13例Castleman病患者的临床及影像学资料,分析其影像表现。结果 (1)肿瘤来源,7例位于腹部,3例位于胸部,2例位于颈部;(2)13例中2例为浆细胞型,11例透明管型;(3)11例透明血管型病例中,5例单发肿块,6例单肿块优势伴周围卫星灶;(4)13例患者肿块内见钙化灶5例,2例条状钙化灶;(5)13例患者中,11例患者行CT增强检查,平扫CT值39~51 Hu(平均44.5 Hu),增强扫描动脉期103~196 Hu(平均141.0Hu),11例肿块均为明显强化;(6)11例增强扫描患者中10例患者肿块周围见不同程度增多、扩张血管,肿瘤实质仅2例见少许纤细小血管影。结论 Castleman病MSCT具有一定特异性,综合分析各征象,有助于提高CT诊断水平。
Objective To analyze the MSCT manifestations of Castleman′s disease, summarize the imaging characteristics of the lesions, and search for signs that can help improve the diagnostic level of CT. Methods Clinical and imaging data of 13 Castleman patients who underwent MSCT examination in the Department of Radiology of the First People′ s Hospital of Zhaotong from June 2010 to December 2018 and were confirmed by postoperative pathology were retrospectively collected, and their imaging manifestations were analyzed. Results(1)Tumor source,7 cases of abdominal position, 3 cases in the chest, 2 cases in the neck;(2)2 of the 13 patients were plasma cell type and 11 were transparent tubular type;(3)Among the 11 cases of transparent vascular type, 5 cases had a single mass,and 6 cases had a single mass advantage with peripheral satellite focus;(4)13 cases of patients with mass calcification lesions in 5 cases, 2 cases of strip calcification lesions;(5)of the 13 patients,11 patients underwent CT enhancement examination, plain CT value of about 39~51 Hu(average 44.5 Hu), enhanced scanning arterial phase 103~196 Hu(average 141.0 Hu),11 cases of tumors were significantly enhanced;(6)among the 11 patients with enhanced scanning, 10 patients saw different degrees of increase and dilation of blood vessels around the tumor, and only 2 cases of tumor parenchyma saw a little thin small blood vessel shadow. Conclusion Castleman′s disease has certain specificity in MSCT,and comprehensive analysis of the signs is helpful to improve the diagnostic level of CT.
