摘要
目的:探讨乙酰半胱氨酸联合盐酸氨溴索经支气管肺泡灌洗注入对慢性阻塞性肺疾病急性加重期(AECOPD)的治疗效果。方法:随机选择2018年11月至2019年1月我院收治的AECOPD患者60例,随机分为实验组及对照组各30例,在接受抗炎、化痰等常规治疗同时进行支气管肺泡灌洗(BAL)治疗,实验组接受支气管肺泡灌洗术联合局部注入乙酰半胱氨酸和氨溴索治疗,对照组采用支气管肺泡灌洗术联合局部注入氨溴索治疗,比较两组临床治疗效果。结果:治疗前两组患者炎性指标、血气分析结果比较,差异无统计学意义(P>0.05),治疗后实验组降钙素原(PCT)、C反应蛋白(CRP)、白细胞计数(WBC)水平低于对照组(P<0.05);治疗后实验组酸碱度(pH)、氧分压(PO2)高于对照组(P<0.05),而二氧化碳分压(PCO2)低于对照组(P<0.05);实验组影像学吸收时间、住院天数及住院费用少于对照组(P<0.05)。结论:在AECOPD治疗过程中,盐酸氨溴索联合乙酰半胱氨酸肺泡灌洗可明显促进炎症消退,改善血气指标,缓解临床症状,缩短住院疗程,减少住院费用,具有一定的临床价值。
Objective:To explore the effect of bronchoscopic alveolar lavage combined with local injection of acetylcysteine and ambroxol hydrochloride in the treatment of acute exacerbaton of chronic obstructive pulmonary disease(AECOPD).Methods:60 hospitalized patients with AECOPD in our hospital from November 2018 to January 2019 were divided into the control group(n=30)and the treatment group(n=30).The treatment group received bronchoscopic alveolar lavage combined with local injection of acetylcysteine and ambroxol hydrochloride,and the control group received bronchoscopic alveolar lavage combined with local injection of ambroxol hydrochloride.The clinical efficacies of the two groups were compared after treatment.Results:The levels of inflammatory indicators and blood gas indexes of the two groups had no statistical difference before treatment(P>0.05).After treatment,the levels of WBC,PCT and CRP in the treatment group were lower than those in the control group(P<0.05).The pH,PCO2,PO2 level of the treatment group improved better than those of the control group(P<0.05).The absorption time,hospital stays and hospitalization expenses of the treatment group were less than those of the control group(P<0.05).Conclusion:Bronchoscopic alveolar lavage combined with local injection of acetylcysteine and ambroxol hydrochloride in the treatment of AECOPD can significantly promote inflammation resolution,improve blood gas indexes,relieve clinical symptoms,and reduce hospital costs and hospital stay,and is worth of clinical promotion.
