摘要
目的观察许斌教授运用自拟消瘤汤1号方,治疗甲状腺结节肝郁痰凝证患者临床疗效,并与左甲状腺素钠片治疗组做对比。方法选取2017年10月-2019年2月在辽宁中医药大学附属医院许斌教授专家门诊诊断为甲状腺结节病的患者,将符合诊断标准的甲状腺结节病患者60例随机分成观察组30例和对照组30例,对照组予以左甲状腺素钠片,观察组以中药汤剂自拟消瘤汤1号方加减治疗。观察治疗前后最大结节直径、临床疗效、中医证候积分及证候疗效。观察时间为6个月。结果治疗前后甲状腺结节最大直径比较,两组结节均缩小,观察组明显优于对照组(P<0.01);治疗后,观察组总有效率高于对照组;两组中医证候积分均明显下降,观察组低于对照组(P<0.01);治疗前中医证候疗效观察组总有效率高于对照组。结论基于中医辨证论治理论,由许斌教授自拟消瘤汤1号方加减治疗肝郁痰凝型甲状腺结节,能缩小甲状腺结节、提高临床疗效、改变中医证候积分及证候疗效,治疗效果优于左甲状腺素钠片,能从整体上改善患者生活质量。
Objective To observe the clinical effect of Professor XU Bi’s self-made Xiaoliu Decoction No.1 in the treatment of patients with thyroid nodules of liver depression and phlegm coagulation type and compare with the treatment of levothyroxine sodium tablets.Methods From October 2017 to February 2019,60 patients with thyroid sarcoidosis who were diagnosed as thyroid sarcoidosis at the outpatient clinic of Professor XU Bin of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were selected,and the patients were randomly divided into experimental group(n=30)and control group(n=30).The control group was given oral levothyroxine sodium tablets,and the experimental group was treated with traditional Chinese medicine self-made Xiaoliu Decoction No.1.Changes in the data of the four groups of maximum nodule diameter,clinical efficacy,TCM syndrome score and syndrome efficacy were observed before and after treatment.The observation period was 6 months.Results Compared with the maximum diameter of thyroid nodules before and after treatment,the nodules of the two groups were reduced.The experimental group was significantly better than the control group and the difference was statistically significant(P<0.01).Comparing the clinical efficacy before and after treatment,the total effective rate of the experimental group was better than that of the control group.Compared with the TCM syndrome scores before and after treatment,the TCM syndrome scores of the two groups were significantly decreased,but the experimental group’s was better than that of the control group,and the difference was statistically significant(P<0.01).Comparing the efficacy of traditional Chinese medicine syndromes before and after treatment,the total effective rate in the experimental group was better than that of the control group.Conclusion Based on the theory of syndrome differentiation and treatment of traditional Chinese medicine,the treatment of thyroid nodules of liver depression and phlegm coagulation type by Professor XU Bi’s self-made Xiaoliu Decoction No.1 can reduce thyroid nodules,improve clinical efficacy,and change TCM syndrome scores and syndrome efficacy.The treatment effect is better than that of levothyroxine sodium tablets,which can improve the quality of life of patients as a whole.
