摘要
目的:通过Meta分析方法检验下肢静脉性溃疡的中西医结合治疗疗效,继而提出最佳的临床治疗方案。方法:通过计算机检索近20年(1999年1月~2019年12月)国内数据库,包括中国期刊全文数据库(CNKI)、维普中文科技期刊全文数据库(VIP)、万方数据库(Wanfang)、中国生物医学文献服务系统(CBM)。依据纳入、排除标准进行文献筛选,采用Cochrane风险评估工具对纳入文献进行质量评估,二分类方法进行数据提取,运用RevMan 5.3软件进行数据统计分析。结果:最终纳入11篇文献,总计1576例患者,治疗组和对照组分别为804、772例。Meta分析结果显示,纳入文献无异质性差异,偏倚较小,中西医结合治疗下肢静脉性溃疡总有效率[RR=1.12,95%CI(1.09,1.15)]较高,明显优于单纯西医治疗。结论:通过数据分析得出,即在研究人群中,中西医结合治疗在下肢静脉性溃疡中疗效显著。弥补单纯西医治疗的不足,提高治愈率,减少复发率。然而目前缺乏相关高质量文献,因此需要样本量大、实验设计严谨规范、随机双盲的临床研究进一步检验结论。
Objective:To test the efficacy of integrated traditional Chinese and western medicine in the treatment of venous ulcer of lower extremities by Meta analysis,and then put forward the best clinical treatment plan.Methods:Domestic databases in recent 20 years(from January 1999 to December 2019)were searched by computer,including China Journal full-text Database CNKI,VIP Chinese Sci-tech Journal full-text Database VIP,Wanfang Database Wanfang and China Biomedical Literature Service system CBM.The literature was screened according to the inclusion and exclusion criteria,the quality of the included literature was evaluated by Cochrane risk assessment tool,the data were extracted by two-classification method,and the data were statistically analyzed by RevMan 5.3 software.Results:A total of 1576 patients were included in 11 literature,including 804 patients in the treatment group and 772 patients in the control group.The results of Meta analysis showed that there was no heterogeneity in the included literature with less bias.The total effective rate of integrated traditional Chinese and Western medicine in the treatment of venous ulcer of lower extremities(risk ratio RR=1.12595%confidence interval CI=1.09 mL 1.15)was higher,which was significantly better than that of western medicine alone.Conclusion:Through the data analysis,it is concluded that in the study population,the combination of traditional Chinese and Western medicine is effective in the treatment of venous ulcer of lower extremities,which makes up for the deficiency of simple western medicine treatment,and improve the cure rate and reduce the recurrence rate.However,there is a lack of relevant high-quality literature,so clinical studies with large sample size,rigorous and standardized experimental design and randomized double-blind are needed to further test the conclusions.
