量子点荧光免疫法测定全血超敏肌钙蛋白Ⅰ与电化学发光法测定血浆超敏肌钙蛋白T的一致性分析 被引量:1

摘要 目的对量子点荧光免疫法测定全血超敏肌钙蛋白Ⅰ(hypersensitivity cardiac troponinⅠ,hs-cTnⅠ)与电化学发光法测定血浆超敏肌钙蛋白T(hypersensitivity cardiac troponin T,hs-cTnT)进行一致性分析.方法2017年8~11月于无锡市第二人民医院急诊检验科连续收集415例肝素抗凝标本,先用抗凝全血在南京诺唯赞的AFS-1000仪器上进行hs-cTnⅠ检测,标本离心后取血浆在罗氏cobas e601分析仪进行hs-cTnT检测.以量子点免疫荧光法检测hs-cTnⅠ作为待考核方法,以电化学发光法检测hs-cTnT作为比对方法,计算两种检测方法结果的相关性和一致性.结果相关性分析结果发现,两种方法结果相关系数r=0.935,P<0.001;其中hs-cTnT>100 ng/L共有45例,与hs-cTnⅠ的相关系数r=0.919;以hs-cTnT>100 ng/L为阳性界值,hs-cTn1>0.10 ng/ml为阳性界值做一致性检验,K值为0.75;Bland-Altman分析显示,415例中有396个点在界限范围之内,19个点在界限范围之外,符合95%数据在范围内的规定.结论量子点免疫荧光法检测hs-cTnⅠ与电化学发光法检测hs-cTnT具有良好的相关性,能够为临床提供一致的检测结果. Objective To compare the quantum dot immunofluorescence method in the detection of whole blood hypersensitivity troponin Ⅰ(hs-cTnⅠ)with electrochemiluminescence method in the detection of plasma hypersensitivity troponin T(hs-cTnT).Methods From August to November 2017,415 cases of heparin anticoagulant samples were collected continuously in the Emergency Test Department of the Second People’s Hospital of Wuxi City.The hs-cTnⅠ test was performed on the AFS-1000 instrument of Nanjing Vazyme with anticoagulated whole blood.After centrifugation,the plasma was taken for detection of hs-cTnT on Roche cobas e601 analyzer.The hs-cTnⅠ was detected by quantum dot immunofluorescence as the evaluation method and the hs-cTnT was detected by electrochemiluminescence as the comparison method;the correlation and consistency of the results of the two detection methods were calculated.Results The correlation coefficient of the two methods was r=0.935,P<0.001;45 cases had hs-cTnT>100 ng/L and the correlation coefficient with hs-cTnI was r=0.919.With hs-cTnT>100 ng/L as the positive boundary value due hs-cTnⅠ>0.10 ng/ml as the positive boundary value for consistency test,K value was 0.75;Bland-Altman analysis showed that 396 points out of the 415 cases were within the limit range and 19 points outside the limits,95%of the data was within the scope of the specification.Conclusion The detection of hs-cTnⅠ by quantum dot immunofluorescence detection has a good Cardiovascular Research,June 2020,Vol.18,No.6 correlation with the detection of hs-cTnT by electrochemiluminescence detection,which can provide the consistent detection results in clinical practice.
