摘要
目的研究绝经激素治疗(MHT)联合降压药对围绝经期高血压的影响。方法收集2017年4月至2019年6月嘉兴市第一医院门诊的围绝经期原发性高血压患者110例,按随机数字表法分为两组,即降压药组(对照组)51例和MHT联合降压药组(治疗组)52例。对照组予比索洛尔5mg,1次/d,口服;治疗组在对照组的基础上加用芬吗通1片,1次/d,疗程12周。分别于治疗后1、2及3个月监测血压,治疗前后检测血管紧张素Ⅱ(AngⅡ)、前列环素(PGI2)、雌二醇(E2)及卵泡刺激素(FSH)水平。结果治疗后2个月及3个月时两组收缩压比较均有统计学差异(均P<0.05);两组舒张压各个时间点的比较均无统计学差异(均P>0.05)。治疗后两组AngⅡ比较无统计学差异(P>0.05),而PGI2、E2、FSH比较均有统计学差异(均P<0.05)。结论MHT联合降压药有助于降低围绝经期高血压患者的收缩压,并能升高围绝经期高血压患者的PGI2水平。
Objective To evaluate the efficacy of menopause hormone therapy combined with antihypertensive drugs for patients with perimenopausal hypertension.Methods A total of 110 patients with perimenopausal essential hypertension were treated in the outpatient department from April 2017 to June 2019 were randomly assigned into two groups,patients in control group(n=51)received bisoprolol 5 mg orally q.d and patients in study group(n=52)received bisoprolol 5 mg and 1 tablet of fenmodone orally q.d for 12 weeks.Blood pressure was monitored at 1,2 and 3 months after treatment.AngiotensinⅡ,prostacyclin,estradiol and follicle stimulating hormone levels were measured before and after treatment.Results There was a significant difference in systolic blood pressure at 2 and 3 months after treatment between the two groups(P<0.05),while there was no significant difference in diastolic blood pressure between the two groups(P>0.05).After treatment,there was no statistical difference in AngⅡlevel(P>0.05),but there were statistical differences in PGI2,E2,and FSH levels between the two groups(P<0.05).Conclusion Menopause hormone therapy combined with antihypertensive drugs can reduce the systolic blood pressure in patients with perimenopausal hypertension and increase the level of PGI2 in patients with perimenopausal hypertension.
