摘要
目的:探讨彩色多普勒超声(color Doppler sonography,CDS)在以旋股外侧动脉斜支为蒂的股前外侧皮瓣穿支血管术前定位中的应用价值。方法:对66例创面行股前外侧皮瓣修复,其中足踝部创面19例,前臂创面18例,手腕部创面29例。软组织缺损面积33×8cm^6×4cm。均有骨或肌腱等深部组织外露。共切取66块皮瓣,其中分叶皮瓣12例。术中观察彩色多普勒超声术前定位旋股外侧动脉斜支的准确性。结果:66例旋股外侧动脉斜支定位,46例存在旋股外侧动脉斜支,17例术中未探查到,而采用旋股外侧动脉降支为蒂的股前外侧皮瓣修复,定位准确率95.5%。66例皮瓣全部存活,随访6~18个月,皮瓣色泽、质地良好,无破溃,无明显色素沉着,外形良好,皮瓣供区下肢的髋膝关节功能无影响。结论:彩色多普勒超声可显示旋股外侧动脉斜支的血供来源、走行方向、体表定位,是一种简单易行、准确性高、对患者无损伤、重复性高的定位方法。
Objective To investigate the application value of color Doppler ultrasound(CDS)in preoperative localization of anterior lateral femoral flap with oblique branch of lateral spiral femoral artery.Methods 66 cases of anterior lateral femoral flap were repaired,including 19 cases of ankle wound,18 cases of forearm wound and 29 cases of wrist wound.soft tissue defect area 33×8cm^6×4cm.All have deep tissue such as bone or tendon exposed.A total of 66 flaps were cut,of which 12 were lobulated.Intraoperative observation of color Doppler ultrasound preoperative localization accuracy of the oblique branch of the lateral spiral femoral artery.Results 66 cases of lateral spiral artery oblique branch located,46 cases of lateral spiral femoral artery oblique branch,17 cases did not explore,but used lateral spiral femoral artery descending branch of anterior lateral femoral flap repair,the accuracy of positioning 95.5%.All 66 cases of flaps survived,followed up for 6 to 18 months,the skin flaps had good color and texture,no rupture,no obvious pigmentation,good appearance,and no effect on the hip and knee function of the lower extremities of the flap donor area.Conclusion Color Doppler ultrasound can show the blood supply source,walking direction and body surface location of the oblique branch of the lateral spiral femoral artery,which is simple and easy to use,and has high accuracy.No injury to patients,high repeatability of the localization method.
