摘要
目的对近年来针灸治疗更年期抑郁症的取穴规律进行分析,为更年期抑郁症的针灸临床治疗提供参考。方法在中国知识资源总库(CNKI)、中国学术期刊数据库(万方数据)、中文科技期刊数据库(重庆维普)检索1983年至2019年间关于针刺治疗更年期抑郁症的所有文献共2173篇,最终将符合标准的58篇文献录入数据库,运用SPSS 20.0、SPSS Modeler 18.0软件进行统计分析。结果针灸治疗更年期抑郁症腧穴使用频次从高到底依次是百会、三阴交、内关、太冲、肾俞、肝俞、心俞,经脉选用以足太阳膀胱经、督脉为主,部位以头部、四肢、背部选穴最多,特定穴以交会穴、背俞穴最为常用,关联规则分析显示相关性较高的穴对有:心俞—肝俞—肾俞、百会—印堂、足三里—三阴交、百会—神门、太冲—合谷,对高频主穴进行聚类分析得到四个聚类群。结论针灸治疗更年期抑郁症疗效确切,穴位以头部、背俞穴和四肢末端为主,符合临床治疗规律;通过数据挖掘技术有助于探索针灸治疗更年期抑郁症的取穴规律,可在一定程度上为针灸治疗更年期抑郁症的临床选穴提供一定的参考依据。
Objective To analyze the rules of acupoint selection in the treatment of climacteric depression with acupuncture and moxibustion in recent years,so as to provide reference for the clinical treatment of climacteric depression.Methods 2173 articles related to treating climacteric depression with acupuncture from 1983-2019 were retrieved from the Chinese National Knowledge Infrastructure(CNKI),China Science Periodical Database(Wanfang Data),and Chinese Scientific Journal Database(Chongqing Weipu).58 articles meet the standards of this research,and are analyzed with SPSS 20.0 and SPSS Modeler 18.0 software.Results The frequency of acupoints used in the treatment of climacteric depression is Baihui(GV20),Sanyinjiao(SP6),Neiguan(PC6),Taichong(LR3),Shenshu(BL23),Ganshu(BL18)and Xinshu(BL5).The main selected meridians are bladder meridian and the governor channel.Additionally,the most commonly used acupoints are located on the head and upper limbs,and the most used specific points are crossng point and back-shu point.The results of association rule analysis show that the highly related points are Xinshu(BL15)-Ganshu(BL18)-Shenshu(BL23),Baihui(DU20)-Yintang(EX-HN3),Zusanli(ST36)-Sanyinjiao(SP6),Baihui(DU20)-Shenmen(HT7),Taichong(LR3)-Hegu(LI4).There were 4 clusters obtained by cluster analysis of high frequency main points.Conclusion Acupuncture and moxibustion are effective in treating climacteric depression.The acupoints are mainly located on head,back and extremities,which are in line with clinical treatment.Data mining technology is helpful to explore the acupuncture and moxibustion treatment of climacteric depression,which can provide evidence for clinical application.
