摘要
目的探讨与分析肝硬化背景下超声造影(CEUS)对早期肝癌及增生结节的诊断及价值。方法选择肝硬化合并局灶性病灶患者60例,病理诊断为肝增生结节18例(结节组)和肝癌42例(肝癌组)。所有患者都给予超声造影检查,记录造影特征并判断诊断价值。结果肝癌组超声造影动脉期、门脉期、延迟期的增强强度与结节组对比差异都有统计学意义(P<0.05)。肝癌组的超声造影起始时间、达峰时间等都显著低于结节组(P<0.05),峰值强度、曲线尖度与曲线下面积都显著高于结节组(P<0.05)。超声造影诊断为肝增生结节20例,肝癌40例,超声造影对肝癌及增生结节的诊断敏感性与特异性为94.4%和92.9%。结论肝硬化背景下超声造影可通过识别增强模式和定量造影参数等,提高早期肝癌及增生结节的鉴别诊断价值。
Objective To investigate and analysis the values of contrast-enhanced ultrasound(CEUS)in the diagnosis of early liver cancer and hyperplastic nodules.Methods 60 patients with cirrhosis and focal lesions were selected and the pathological diagnosis were 18 patients of liver hyperplasia(nodular group)and 42 patients of liver cancer(hepatocarcinoma group).All patients were underwent contrast-enhanced ultrasound examination to record the contrast characteristics and judge the diagnostic value.Results The contrast-enhanced intensity of the arterial phase,portal phase,and lag phase in the hepatocarcinoma group compared to the nodular group were statistically significant difference(P<0.05).The onset time and peak time of contrast-enhanced ultrasound in the liver cancer group were significantly lower than those in the nodule group(P<0.05),and the peak intensity,curve tip and area under the curve were significantly higher than those in the nodule group(P<0.05).Contrast-enhanced ultrasonography were diagnosed as 20 patients of hepatic hyperplasia,40 patients of liver cancer,and the sensitivity and specificity of contrast-enhanced ultrasound for liver cancer and hyperplastic nodules were 94.4%and 92.9%.Conclusion Contrast-enhanced ultrasonography in the based of cirrhosis can improve the differential diagnosis value of early liver cancer and hyperplastic nodules by identifying enhanced patterns and quantitative contrast parameters.
