摘要
目的:探讨锁定钢板外置治疗胫骨骨折术后感染的临床疗效。方法:选取2014年1月-2018年7月在本院接受治疗的胫骨骨折术后感染患者50例,按随机数字表法分成研究组与对照组,每组25例。研究组采用锁定钢板外置治疗,对照组采用传统的外固定架治疗。比较两组感染控制、愈合情况、关节活动度、美国特种外科医院(hospital special surgery,HSS)评分、并发症及手术费用。结果:所有患者随访12~36个月,两组术后感染控制率比较,差异无统计学意义(P>0.05)。研究组延迟愈合率低于对照组,且愈合时间短于对照组(P<0.05)。术前与术后6个月,两组膝关节屈曲度、伸直度及HSS评分比较,差异均无统计学意义(P>0.05)。两组术后6个月HSS评分均高于术前(P<0.05)。研究组并发症发生率于手术费用均低于对照组(P<0.05)。结论:锁定钢板外置治疗胫骨骨折感染能够有效控制感染,减少延迟愈合,恢复膝关节活动度,是一种有效的替代传统外固定架的外固定治疗方法。
Objective:To explore the clinical efficacy of external locking plate in the treatment of postoperative infection of tibial fracture.Method:A total of 50 patients with postoperative tibial fracture infection treated in our hospital from January 2014 to July 2018 were selected.They were divided into study group and control group according to the random number table method,25 patients in each group.The study group was treated with external locking plate and the control group was treated with conventional external fixation.Infection control,healing,range of motion,hospital special surgery(HSS)score,complications and surgical costs were compared between the two groups.Result:All the patients were followed up for 12 to 36 months,there was no statistically significant difference in the postoperative infection control rate between the two groups(P>0.05).The rate of delayed healing in the study group was lower than that in the control group,and the healing time was lower than that in the control group(P<0.05).There were no statistically significant differences in knee flexion,straightening degree and HSS score between the two groups before and 6 months after surgery(P>0.05).HSS scores of both groups at 6 months after surgery were higher than those before surgery(P<0.05).The incidence of complications and surgical costs in the study group were lower than those in the control group(P<0.05).Conclusion:External locking plate in the treatment of tibial fracture infection can effectively control the infection,reduce the delayed healing and restore the range of motion of the knee joint.It is an effective external fixation method to replace traditional external fixator.
