摘要
目的:研究腹腔镜下宫颈峡部环扎术治疗宫颈功能不全的临床效果及对围生期结局的影响。方法:选取2017年10月-2018年10月本院收治的76例宫颈功能不全的患者为研究对象。按照随机数字表法将其分为研究组和对照组,各38例。对照组行经阴道宫颈峡部环扎术,研究组行腹腔镜下宫颈峡部环扎术,比较两组手术情况、术后住院时间、并发症发生情况、妊娠期宫颈长度及围生儿体质量、妊娠情况。结果:研究组住院时间、手术时间均短于对照组,术中出血量少于对照组,差异均有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05);研究组治疗成功率高于对照组(P<0.05);研究组妊娠期宫颈长度长于对照组,围生儿体质量高于对照组,差异均有统计学意义(P<0.05)。结论:腹腔镜下宫颈峡部环扎术治疗宫颈功能不全可以节约手术时间,减少术中出血,利于术后恢复,对于胎儿在母体的生长提供更好的环境,不会增加术后并发症,改善了妊娠结局,可以在临床上推广。
Objective:To investigate the clinical effect of laparoscopic cervical isthmus cerclage in the treatment of cervical insufficiency and its effect on perinatal outcome.Method:A total of 76 patients with cervical insufficiency treated in our hospital from October 2017 to October 2018 were selected as the research objects.According to the random number table,they were divided into study group and control group,38 cases in each group.The control group received transvaginal cervical isthmus cerclage,and the study group received laparoscopic cervical isthmus cerclage.The operation condition,postoperative hospital stay,complications,cervical length during pregnancy,perinatal body weight and pregnancy were compared between the two groups.Result:The time of hospitalization and operation in the study group were shorter than those in the control group,and the amount of bleeding in the operation was less than that in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).The treatment success rate of the study group was higher than that of the control group(P<0.05).The cervical length during pregnancy in the study group was longer than that in the control group,and the perinatal body weight was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Laparoscopic cervical isthmus cerclage for the treatment of cervical insufficiency can save operation time,reduce intraoperative bleeding,facilitate postoperative recovery,provide a better environment for the growth of the fetus in the mother’s body,do not increase postoperative complications,improve pregnancy outcome,and can be popularized in clinical practice.
