摘要
目的探讨心脏彩超三血管序列切面对胎儿先天性心脏病的诊断价值。方法采用回顾性分析的方法,采集南阳市第一人民医院2018年6月-2019年5月经手术结果证实为先天性心脏病胎儿120例临床资料,孕妇均行心脏彩超三血管序列切面检查,以手术结果为金标准,评估心脏彩超三血管序列切面的诊断准确率,并统计胎儿先天性心脏病三血管序列切面异常类型。结果室间隔缺损10例,右室发育不良8例,左室发育不良11例,右室双出口2例,法洛四联症4例,三尖瓣下移畸形4例,二尖瓣闭锁2例,三尖瓣闭锁1例,肺动脉闭锁3例,单纯肺动脉狭窄9例,主动脉弓离断2例,主动脉缩窄4例,大动脉转位7例,主动脉狭窄6例,完全性房室间隔缺损8例,血管环3例,下腔静脉离断8例,永存动脉干6例,永存动瘤4例,动脉导管狭窄2例,心脏肿瘤3例,心肌钙化4例,永存左上腔静脉9例;与手术结果相比,三血管序列切面诊断率为76.67%。结论三血管序列切面在初步确诊先天性心脏病胎儿中有一定诊断价值,可大大提高胎儿超声心动图检查的准确率。
Objective To explore the diagnostic value of three vessel serial of color Doppler echocardiography in fetal congenial heart disease.Methods Through retrospective analysis, clinical data of 120 fetus with congenital heart disease from July,2018 to May,2019 in Nanyang First People’s Hospital was collected. Pregnant women received three vessel serial section of color Doppler echocardiography,and the operation results were regarded as the gold standard. The accuracy rate of three vessel serial section of color Doppler echocardiography was evaluated,and abnormal types were recorded.Results There were 10 cases of VSD, 8 cases of right ventricular dysplasia, 11 cases of left ventricular dysplasia, 2 cases of right ventricular double outlet, 4 cases of tetralogy of Fallot, 4 cases of tricuspid valve downward deformity, 2 cases of mitral valve atresia, 1 case of tricuspid valve atresia,3 cases of pulmonary artery atresia,9 cases of simple pulmonary artery stenosis,2 cases of aortic arch disconnection,4 cases of aortic coarctation,7 cases of transposition of great arteries,and 6 cases of aortic stenosis. There were 8 cases of complete atrioventricular septal defect,3 cases of vascular rings,8 cases of inferior vena cava disconnection,6 cases of permanent arterial trunk,4 cases of permanent aneurysm,2 cases of artery stenosis,3 cases of cardiac tumor,4 cases of myocardial calcification and 9 cases of permanent left superior vena cava. Compared with the results of operation,the diagnostic rate of three vessel sequence was 76.67%.Conclusion The three vessel serial section has certain diagnostic value in the initial diagnosis of congenital heart disease,which can greatly improve the accuracy of fetal echocardiography.
