骨科手术器械采用密闭式硬质灭菌盒包装和灭菌的效果分析 被引量:4

摘要 目的基于复用型骨科手术器械在包装和灭菌等环节存在的问题,通过差异化的包装和灭菌对比试验,寻求骨科手术器械包装和灭菌的有效措施。方法从2016年6月开始,将我院中心手术室和急诊科手术室各4套骨科手术器械包作为试验样本,分别设为试验组和对照组,试验组采用密闭式硬质灭菌盒进行密封包装,对照组采用先装篮筐后用灭菌包布闭合包装;2组器械经灭菌技术存在差异的A、B两台压力蒸汽灭菌器进行灭菌处理,在灭菌出锅和临床使用器械环节,依次检测灭菌湿包发生率、灭菌包装失效率、灭菌效果和器械非正常坏损情况;试验组和对照组分别试验432包次(22464件次)和306包次(15912件次)。结果本研究2组器械均达到可靠的灭菌效果,但试验组器械经A、B两台灭菌器灭菌处理后湿包发生率分别为10%和0,差异有显著统计学意义(P<0.01);对照组器械经A、B两台灭菌器灭菌处理后湿包发生率分别为7.5%和2.3%,差异无统计学意义(P>0.05);试验组和对照组骨科手术器械灭菌后的灭菌包装失效率分别为0和4.2%,差异有显著统计学意义(P<0.01);试验组和对照组骨科手术器械灭菌后的非正常坏损率分别为0.02%和0.39%,差异有显著统计学意义(P<0.01)。结论骨科手术器械采用密闭式硬质灭菌盒灭菌包装的效果显著优于传统的“篮筐+包布”的闭合式灭菌包装,但需采用合适的灭菌技术以防止发生灭菌湿包。 Objective To explore the effective measures for packaging and sterilization of orthopedic surgical instruments by comparing differential packaging and sterilization based on the problems existing in the packaging and sterilization of reusable orthopedic surgical instruments.Methods Since June 2016,Four sets of orthopedic surgical instruments in the central operating room and emergency operating room of our hospital were taken as test samples,and they were respectively set as test group and control group.The test group was sealed with closed hard sterilization box,and the control group was sealed with basket first and then closed with sterilization cloth.Both two groups were sterilized by two sets of pressure steam sterilizers(A and B)with different sterilization technology.In the process of sterilization out of the pot and clinical use of instruments,the occurrence rate of sterilization wet package,failure rate of sterilization package,sterilization effect and abnormal damage of instruments were detected successively;432 packages(22464 packages)and 306 packages(15912 packages)were tested in the test group and the control group respectively.Results Both two groups of instruments achieved reliable sterilization effect,but the wet package rate of the test group were 10%and 0 after sterilized by A and B sterilizers,the difference was statistically significant(P<0.01),and the wet package rate of the control group were 7.5%and 2.3%after sterilized by A and B sterilizers,the difference was not statistically significant(P>0.05).The sterilization package failure rate of the test group and the control group were 0 and 4.2%respectively,the difference was statistically significant(P<0.01).The abnormal damage rate of the test group and the control group were 0.02%and 0.39%respectively,the difference was statistically significant(P<0.01).Conclusion Sterilization effect of closed rigid sterilization box for orthopedic surgical instruments is significantly better than that of traditional"basket plus cloth"closed sterilization package,but appropriate sterilization technology should be adopted to prevent the generation of sterilization wet package.
