摘要
目的:分析营养状况、是否合并糖尿病对慢性阻塞性肺疾病机械通气患者脱机、预后的影响。方法:根据2018年GOLD指南,搜集笔者所在医院RICU、ICU中进行有创机械通气的COPD患者、性别、年龄、ALB、BMI等相关资料,以及是否合并糖尿病,对成功脱机组、呼吸机依赖组、死亡组的患者相关因素水平进行比较、了解其有无统计学差异。结果:共纳入152例患者,其中死亡组28例,呼吸机依赖组49例及成功脱机组75例,与呼吸机依赖组及死亡组相比,成功脱机组ALB显著高(P<0.05或<0.01);与呼吸机依赖组相比,死亡组及成功脱机组BMI显著高(P<0.05或<0.01),各组糖尿病患病率差异无统计学意义(P>0.05)。结论:COPD患者的ALB、BMI越高,则越可能脱机成功,但BMI过高,可能导致死亡,合并糖尿病与否对脱机结果影响不大。
Objective:To analyze the influence on weaning and prognosis of patients with mechanical obstruction of chronic obstructive pulmonary disease nutritional status and whether with diabetes.Method:According to COLD guideline made in 2018,COPD patients with invasive mechanical ventilation in our hospitals were collected,gender,age,ALB,BMI,etc,and whether they had diabetes were collected.The patients’ relevant factors were compared in the successful weaning group,ventilator dependent group,death group to see if they were statistically different.Result:A total of 152 patients were included,the death group was 28 cases,the ventilator dependent group was 49 cases,and the successful weaning group was 75 cases.Compared with the ventilator dependent group and the death group,the ALB of the successful weaning group was significantly higher (P<0.05 or P<0.01),and the BMI of the death group and the successful weaning group were significantly higher than that of the ventilator dependent group (P<0.05 or P<0.01).Diabetes mellitus rate had no significant difference in three groups (P>0.05).Conclusion:The higher the ALB and BMI of COPD patients,the more likely they are to be successful weaning,but the BMI is too high,which may lead to death.The combination of diabetes or not has little effect on results.
