摘要
目的:探究彩色多普勒超声在诊断胎盘植入中的应用价值,分析胎盘植入的影响因素。方法:选取2016年6月-2019年6月笔者所在医院接收的50例疑似胎盘植入孕妇,均进行彩色多普勒超声检查及病理检查,并对年龄、流产史、剖宫史及脐动脉血流S/D值等信息进行统计。结果:病理检出胎盘植入11例,正常39例。彩色多普勒超声检出胎盘植入10例,正常40例,准确率为94.0%,灵敏度为81.8%,特异性为97.4%。胎盘植入孕妇年龄、脐动脉血流S/D值均大于正常孕妇,有流产史及剖宫产史均多于正常孕妇,差异均有统计学意义(P<0.05)。孕妇年龄、流产史、剖宫产史及脐动脉血流S/D值均是胎盘植入的影响因素,差异均有统计学意义(P<0.05)。结论:彩色多普勒超声检查对诊断胎盘植入的价值较高,具有较高的诊断准确率,值得在临床中推广。同时,临床应高度关注年龄较大、具有流产史及剖宫产史的孕妇,尽可能降低胎盘植入发生风险。
Objective:To explore the application value of color Doppler ultrasound in the diagnosis of placenta implantation,and analyze the influencing factors of placenta implantation.Method:A total of 50 pregnant women with suspected placenta implantation received by our hospital from June 2016 to June 2019 were selected,and they were examined by color Doppler ultrasound and pathological examination.Age,history of abortion,history of caesarean section,and S/D value of umbilical artery hemodynamics were calculated.Result:Pathological examination showed that 11 cases of placenta implantation and 39 cases of normal.Color Doppler ultrasound examination showed that 10 cases of placenta implantation and 40 cases of normal,and the accuracy was 94.0%,sensitivity was 81.8%,and specificity was 97.4%.The age and S/D value of umbilical artery hemodynamics of pregnant women with placenta implantation were greater than those of normal pregnant women,and history of abortion and history of caesarean section were more than those of normal pregnant women,and the differences were statistically significant (P<0.05).The age,history of abortion,history of caesarean section,and S/D value of umbilical artery hemodynamics were all influencing factors of placenta implantation,and the differences were statistically significant (P<0.05).Conclusion:Color Doppler ultrasound has a high value in the diagnosis of placenta implantation and has high diagnostic accuracy,which is worthy of extensive promotion in clinical practice.At the same time,clinical attention should be paid to pregnant women who are older,with history of abortion and cesarean section to minimize the risk of placenta implantation.
