摘要
目的评价经括约肌段瘘管剔除加材料修补术治疗高位肛瘘的临床疗效及安全性。方法选取2015年1月至2016年6月诊断为高位单纯性肛瘘患者72例。随机分为两组(试验组和对照组),每组36例。试验组行经括约肌段瘘管剔除加材料修补术。对照组采用传统“切开挂线”法治疗。观察两组术后疼痛评分、创面愈合时间、临床疗效评定,术后1年肛门功能评价、术后复发情况。结果试验组术后6 h、第1天、第7天换药疼痛的视觉模拟疼痛评分(VAS)低于对照组(P<0.05)。试验组创口平均愈合时间、临床疗效、术后1年对肛门功能评估优于对照组(P<0.05)。两组术后1年随访复发率差异无统计学意义(P>0.05)。结论经括约肌段瘘管剔除加材料修补术治疗高位单纯性肛瘘临床疗效可靠,肛门功能保护良好,值得临床推广应用。
Objective To evaluate the clinical efficacy and safety of intersphincteric fistula tract resection combined with material repair in the treatment of high simple anal fistula.Methods A total of 72 patients diagnosed with high simple anal fistula from January of 2015 to June of 2016 were selected and randomly divided into the experiment group and control group with 36 cases in each group.The experiment group was treated with intersphincteric fistula tract resection combined with material repair,while the control group was treated with the incision-thread-drawing procedure.The postoperative visual analogue scale(VAS)score,wound healing time and clinical efficacy and the anal function and recurrence rate at 1 year after operation were observed.Results The postoperative VAS scores of dressing change pain at 6 hours,1 day and 7 days after operation were significantly lower in the experiment group than in the control group(P<0.05).The postoperative average wound healing time was shorter and the clinical efficacy and the anal function at 1 year after operation were better in the experiment group than in the control group,and the differences were statistically significant(P<0.05).There were no significant differences between the two groups in the recurrence rate at 1 year after operation(P>0.05).Conclusion The removal of sphincter segment fistula combined with material repair is effective and reliable in the treatment of high simple anal fistula with good protection of anal function,so it is worthy of clinical promotion and application.
