摘要
目的分析和预防临床抗菌药物不合理应用导致的不良反应。方法回顾性分析连云港市赣榆区第二人民医院2017年1月至2018年10月收治的500例不合理应用抗菌药物导致不良反应患者的临床资料,从抗菌药物种类、不合理应用方式、给药途径及累及部位分析不合理应用抗菌药物导致不良反应发生的情况。结果各类抗菌药物中,应用头孢菌素类不良反应发生率最高,为39.00%;不合理应用抗菌药物致不良反应累及部位前3位依次为皮肤组织、消化系统及神经系统,发生率分别为32.80%、23.40%、17.20%;给药途径中,静脉输液致不良反应发生率最高,为56.60%;不合理应用方式致不良反应发生率从高到低前3位依次为滴速过快、给药时间过长、剂量过大,分别为27.40%、24.80%、21.60%。结论不合理应用头孢菌素类抗菌药物最易引发皮肤组织不良反应,静脉输液且滴速过快是最易导致不良反应的给药方式,可针对性加强预防措施,提高抗菌药物应用的安全性。
Objective To analyze and prevent the adverse reactions caused by unreasonable application of antibacterial drugs.Methods Retrospective analysis of clinical data of 500 patients with adverse reactions caused by unreasonable application of antibacterial drugs in the Second People's Hospital of Ganyu District,Lianyungang City from January 2017 to October 2018,analyze the occurrence of adverse reactions caused by irrational application of antibacterial drugs from the types of antibacterial drugs,unreasonable application methods,routes of administration and affected sites.Results Among all kinds of antibacterial drugs,the incidence of adverse reactions of cephalosporins was the highest,which was 39.00%;The top 3 sites of adverse reactions caused by irrational application of antibacterial drugs were skin tissue,digestive system,and nervous system in turn,with incidence rates of 32.80%,23.40%,and 17.20%,respectively;In the route of administration,the incidence of adverse reactions caused by intravenous infusion was the highest,which was 56.60%;The incidence of adverse reactions caused by unreasonable application methods from high to low was in the order of fast drip rate,long administration time,and excessive dose,which were 27.40%,24.80%,and 21.60%,respectively.Conclusion Improper application of cephalosporin antibacterial drugs is most likely to cause adverse skin tissue reactions.Intravenous infusion and dripping too fast are the most likely to cause adverse reactions.It can be targeted to strengthen preventive measures and improve the safety of antibacterial drugs.
