摘要
目的分析医院药房退药的原因,针对具体的退药原因,制定提高药局药事服务质量的措施,以降低患者的退药率。方法收集2015年1月至2018年10月我院药房退药的452份处方,对退药原因、退药涉及科室等数据、资料进行回顾性分析,并制定针对性的干预措施。结果从退药涉及科室中看出,内科退药率最高,其次为儿科、急诊科;从退药原因上来看,退药原因主要包括四方面:其一是患者自身因素,主要体现在患者用药依从性差;其二是药物的不良反应因素;其三是医师因素,主要体现在临床用药不合理;其四是其他因素,主要体现在药房工作纰漏、药品质量差、收费因素等方面。针对医院药房退药的原因,从以下几方面制定改进方案:第一,要重视对药品常见不良反应的管理,降低临床用药的不良反应发生率;第二,要不断更新药局药师服务理念,提升临床用药的合理性;第三,要加强对药房工作人员的培训和职业教育,以提升药房的服务质量;第四,要实现药房工作机制的不断革新,提升药师的专业素养;第五,规范退药的相关流程。结论退药现象在医院的日常工作中较为常见,对此,医院药局要对自身进行改革,加大药师的培训力度,加强医患之间的沟通和交流,以提高临床用药的合理性,最大程度上降低退药率。
Objective To analyze the reasons for drug withdrawal in hospital pharmacies,and to develop measures to improve the quality of pharmacy services for specifi c drug withdrawal reasons,so as to reduce the drug withdrawal rate.Methods A total of 452 prescriptions for drug withdrawal from our hospital from January 2015 to October 2018 were collected.The data and data of the reasons for drug withdrawal and drug withdrawal were retrospectively analyzed,and targeted interventions were developed.Results From the department of drug withdrawal,the rate of drug withdrawal is the highest,followed by pediatrics and emergency department.From the point of view of drug withdrawal,the reasons for drug withdrawal mainly include four aspects:one is the patient's own factors,mainly refl ected in the patient's medication compliance.The second is the adverse reaction factors of the drug;the third is the doctor factor,which is mainly refl ected in the unreasonable clinical drug use;the fourth is other factors,mainly refl ected in the pharmacy work leakage,poor drug quality,and charging factors.In view of the reasons for drug withdrawal in hospital pharmacies,we will formulate improvement plans from the following aspects:First,we must pay attention to the management of common adverse reactions of drugs and reduce the incidence of adverse reactions of clinical drugs.Second,we must constantly update the pharmacy service concept of pharmacies.To improve the rationality of clinical medication;third,we must strengthen the training and vocational education of pharmacy staff to improve the quality of pharmacy services;Fourth,we must continue to innovate the pharmacy work mechanism and enhance the professionalism of pharmacists;Regulate the relevant process of withdrawal.Conclusion The drug withdrawal phenomenon is more common in the daily work of the hospital.In this regard,the hospital pharmacy should reform itself,increase the training of pharmacists,and strengthen the communication and communication between doctors and patients to improve the rationality of clinical medication.Minimize the rate of withdrawal.
