摘要
目的:应用大数据思维构建脑卒中“监测与分析模型”,实现脑卒中质量管理从人工化到自动化的转变,从而达到规范卒中诊疗,提升医疗质量的目的。方法:依据科学的质量监测与分析模型,建立了医疗质量监测与分析系统,对脑卒中诊疗过程中的问题进行监测并开展针对性的改进,分析改进前后相关指标的变化情况。结果:分析上线前后数据发现:(1)原因反馈类别:人员配合方面原因占65%,人力资源配置原因占16%,自身业务能力原因占13%,医院自身的硬件资源配置原因占4%,其他原因和人员培训各占1%;(2)不良事件发生率及死亡率变化:上线前半年不良事件发生率为2.6%,上线后半年下降到了2.00%;上线前半年死亡率为0.60%,而上线后半年降至0%;(3)住院平均费用变化:系统上线后半年脑卒中患者的平均治疗费用比上线前半年缩减幅度约8%。结论:通过医疗质量监测与分析系统,提升了脑卒中诊疗的诊疗质量,实现质控管理的数据化、精细化、智能化。
Objective:For the purpose of standardizing the diagnosis and treatment of stroke and improving medical quality by applying large data thinking to design the"monitoring and analysis model"of stroke and realizing the adaptability change of stroke quality management from artificiality to datamation.Methods:The monitoring and analysis system of medical quality was designed according to scientific quality monitoring and analysis model,monitored problem in the process of diagnosis and treatment of stroke and executed pertinent improvement,and analyzed the changes of before and after improved related indexes.Results:By comparing the past and current online data,we found:(1)Cause feedback category:we found that the personnel coordination accounted for 65%of all the reasons,the allocation of human resources accounted for 16%of all the reasons,the Business capability accounted for 13%of all the reasons,the hospital's own hardware resource allocation accounted for 4%of all the reasons,the personnel training and others reasons accounted for 1%of all the reasons.(2)Incidence of adverse events and mortality:the incidence of adverse events in the half year before on-line was 2.6%,down to 2%after on-line,and the death rate in the half year before on-line was 0.60%,down to 0%for half a year after on-line.(3)The change of average cost of hospitalization:the average cost of treatment of stroke patients for half a year after on-line was 8%less than that of the first half of the year.Conclusions:The monitoring and analysis system of medical quality improves the quality of diagnosis and treatment of stroke and can realize digitized,sophisticated and intelligent quality control management.
