摘要
目的探讨血清人类软骨糖蛋白39(YKL-40)和补体C1q肿瘤坏死因子相关蛋白3(CTRP-3)在2型糖尿病(T2DM)患者心血管并发症中的诊断价值。方法选择2018年1月1日至10月30日东莞市清溪医院收治的80例T2DM患者为研究对象,所有患者按有无心血管并发症分为T2DM合并心血管并发症组(n=50)和T2DM无心血管并发症组(n=30)。采用双抗体夹心酶联免疫吸附法检测血清YKL-40和CTRP-3水平,采用二分类反应变量的Logistic回归分析T2DM合并心血管并发症的影响因素,采用受试者工作特征曲线下面积(AUC)评估血清YKL-40、CTRP-3对T2DM心血管并发症的诊断价值。结果T2DM合并心血管并发症组年龄、体质指数(BMI)、腹围、血清血尿酸、肌酐、YKL-40高于T2DM无心血管并发症组[(67±13)岁比(46±10)岁,(42±7)kg/m^2比(38±6)kg/m^2,(92±11)cm比(85±8)cm,(384±106)μmol/L比(284±69)μmol/L,(82±26)μmol/L比(60±14)μmol/L,(7.68±0.75)μg/L比(2.61±0.40)μg/L],CTRP-3低于T2DM无心血管并发症组[(0.60±0.13)μg/L比(1.73±0.17)μg/L](P<0.05)。二分类反应变量的Logistic回归分析结果显示,老龄,高BMI、腹围,血清血尿酸、肌酐、YKL-40水平升高是T2DM患者发生心血管并发症的危险因素(P<0.05),血清CTRP-3水平升高是T2DM患者并发心血管并发症的保护因素(P<0.05)。血清YKL-40、CTRP-3诊断心血管并发症的AUC分别为0.955、0.974,两者联合检测诊断心血管并发症的AUC为1.000。结论血清YKL-40、CTRP-3是T2DM患者发生心血管并发症的影响因素,两者联合检测对T2DM患者发生心血管并发症的诊断价值更高。
Objective To investigate the diagnostic value of serum human cartilage glycoprotein 39(YKL-40)and complement C1q/tumor necrosis factor-related protein-3(CTRP-3)in cardiovascular complications of patients with type 2 diabetes mellitus(T2DM).Methods Totally 80 patients with T2DM in Dongguan Qingxi Hospital from Jan.1st to Oct.30th 2018 were included in the study,according to the existence of cardiovascular complications,they were divided into a T2DM with cardiovascular complication group(n=50)and a T2DM without cardiovascular complication group(n=30).The serum levels of YKL-40 and CTRP-3 were detected by double-antibody sandwich enzyme-linked immunosorbent assay.Logistic regression of binary response variables was used to analyze the influencing factors of cardiovascular complications of T2DM.The diagnostic value of serum YKL-40 and CTRP-3 for cardiovascular complications of T2DM was assessed by using the area under curve(AUC)of the receiver operating characteristic.Results Age,body mass index(BMI),abdominal circumference,serum uric acid,creatinine and YKL-40 in the T2DM with cardiovascular complication group were higher than those in the T2DM without cardiovascular complication group[(67±13)year vs(46±10)year,(42±7)kg/m^2 vs(38±6)kg/m^2,(92±11)cm vs(85±8)cm,(384±106)μmol/L vs(284±69)μmol/L,(82±26)μmol/L vs(60±14)μmol/L,(7.68±0.75)μg/L vs(2.61±0.40)μg/L],and CTRP-3 was lower than that in the control group[(0.60±0.13)μg/L vs(1.73±0.17)μg/L](P<0.05).Logistic regression analysis of binary response variables showed that aging,high BMI,abdominal circumference,elevated serum uric acid,creatinine and YKL-40 were risk factors for cardiovascular complications in T2DM patients(P<0.05),and elevated serum CTRP-3 was a protective factor for cardiovascular complications in T2DM patients(P<0.05).The AUC of serum YKL-40 and CTRP-3 diagnosis of cardiovascular complications was 0.955 and 0.974,respectively,and the AUC of the combination of them was 1.000.Conclusion Serum YKL-40 and CTRP-3 are the influencing factors of cardiovascular complications in T2DM patients,and their combined detection has a higher value for the diagnosis.
