2型糖尿病患者血清胱抑素C与心血管自主神经病变的相关性研究 被引量:10

Correlation between serum cystatin C and cardiovascular autonomic neuropathy in patients with type 2 diabetes

摘要 目的 探讨T2DM患者血清胱抑素C(Cys-C)与心血管自主神经病变(CAN)的相关性.方法 选取2017年4月至2018年10月于东部战区总医院内分泌科住院的T2DM患者386例,根据标准心血管反射试验(CARTs)分为单纯T2DM组(T2DM,n=166)和T2DM合并CAN组(CAN,n=220).检测各组血清Cys-C水平,采用CARTs测定CAN.结果 CAN组血清Cys-C水平高于T2DM组[1.01(0.87,1.20)vs 0.93(0.82,1.10)mg/L,Z=-3.027,P<0.01].Spearman相关分析显示,血清Cys-C水平与立卧位心率差、Valsalva动作R-R比值、深呼吸心率差呈负相关.二元Logistic回归分析结果显示,血清Cys-C是T2DM合并CAN的影响因素.结论 T2DM患者血清Cys-C水平与CAN发生关系密切.对高血清Cys-C水平的T2DM患者应更加关注CAN的发生风险. Objective To investigate the correlation between serum cystatin C(CysC)and cardiovascular autonomic neuropathy(CAN)in type 2 diabetes mellitus(T2DM).Methods From April 2017 to October 2018,386 T2DM patients were selected from the Department of Endocrinology of the General Hospital of Eastern Theater Command.According to the standard cardiovascular reflex test(CARTs),the patients were divided into simple T2DM group(T2DM,n=166)and T2DM combined with CAN group(CAN,n=220).Serum Cys-C levels were measured and CAN was determined by CARTs.Results The serum Cys-C concentration in CAN group was significantly higher than that in T2DM group[1.01(0.87,1.20)vs 0.93(0.82,1.10)mg/L,Z=-3.027,P<0.01].Spearman analysis showed that serum Cys-C was negatively correlated with heart rate variation during deep breathing,lying to standing and Valsalva ratio response to standing.Logistic regression analysis showed that serum Cys-C was an independent risk factor for the development of CAN.Conclusion The level of serum Cys-C in patients with T2DM is positively correlated with the occurrence of CAN.More attention should be paid to the risk of CAN in patients with T2DM with high serum Cys-C level.
