摘要
目的研究缬沙坦对原发性高血压患者心率变异性及左心室后壁舒张末期厚度(LVPWT)、左心室舒张末期内径(LVEDd)的影响。方法回顾性分析2017年1月至2018年12月在江苏省海门市中医院进行诊疗的78例原发性高血压患者,按照治疗方法的不同分为对照组和观察组,各39例。对照组给予马来酸依那普利片治疗,观察组给予缬沙坦胶囊治疗,均连续治疗24周。比较治疗前后2组患者血压、左心室肥厚情况、心率变异性水平差异,比较治疗期间用药安全性。结果2组患者治疗后收缩压(SBP)、舒张压(DBP)、LVPWT、LVEDd、室间隔舒张末期厚度(LVST)水平及低频与高频功率比值(LF/TF)值均较治疗前明显降低,24 h连续全部窦性R-R间期标准差(SDNN)、24 h内每5 min时段窦性R-R间期均值的标准差(SDANN)、连续全部窦性R-R间期的平方根(RMSSD)明显高于治疗前(P<0.05),治疗后2组上述指标差异无统计学意义(P>0.05)。观察组不良反应发生率为5.13%明显低于对照组20.51%(P<0.05)。结论缬沙坦可逆转轻中度原发性高血压患者左心室肥厚,对改善患者心率变异性有显著效果,且不良反应少,安全性较高,值得临床推广应用。
Objective To study the effect of valsartan on heart rate variability,LVPWT and LVEDd in patients with essential hypertension.Methods Totally 78 patients with essential hypertension who were diagnosed and treated in Haimen Hospital of traditional Chinese medicine from January 2017 to December 2018 were analyzed retrospectively.According to different treatment methods,the patients were divided into control group and observation group,each group had 39 cases.The control group was treated with enalapril maleate tablets and the observation group with valsartan capsules for 24 weeks.The difference of blood pressure,left ventricular hypertrophy and heart rate variability between the two groups before and after treatment were compared,and the safety of medication during the treatment was compared.Results The levels of SBP,DBP,LVPWT,LVEDd,LVST and LF/TF in the two groups were significantly lower than those before treatment,and SDNN,SDANN and RMSSD were significantly higher than those before treatment(P<0.05).There was no significant difference between the two groups after treatment(P>0.05).The incidence of adverse reactions in the observation group was 5.13%,which was significantly lower than that in the control group(20.51%)(P<0.05).Conclusion Valsartan can reverse left ventricular hypertrophy in patients with mild to moderate essential hypertension,and it has a significant effect on improving heart rate variability,with fewer adverse reactions and higher safety,which is worthy of clinical application.
