摘要
目的系统评价中西医结合治疗非动脉炎性前部缺血性视神经病变的临床疗效。方法计算机检索2010年1月-2019年2月CNKI、万方、VIP及PubMed等数据库,搜集相关中医药联合西医治疗非动脉炎性前部缺血性视神经病变的临床随机对照试验研究。两位研究者独立筛选文献、提取资料及评价纳入研究的风险偏倚,运用ReVman 5.3软件对所纳入研究进行Meta分析。结果共纳入10项随机对照试验研究,合计733例。Meta分析结果表明,与单纯西医治疗相比,中西医结合治疗非动脉炎性前部缺血性视神经病变能明显提高中心视力值(MD=0.08,95%CI[0.06,0.11],Z=6.03,P<0.01)、平均光敏感度(MD=2.19,95%CI[1.55,2.84],Z=6.66,P<0.01)、总有效率(OR=4.70,95%CI[2.90,7.64],Z=6.26,P<0.01)及降低平均缺损值(MD=-2.64,95%CI[-3.71,-1.58],Z=4.86,P<0.01),两者间差异均有统计学意义(P<0.05)。结论现有检索得到的证据表明,中西医结合治疗非动脉炎性前部缺血性视神经病变的临床疗效明显优于单纯西医治疗,尤其在提高视力、减轻视野缺损方面更为显著。
OBJECTIVE To assess the clinical efficacy of integrated Traditional Chinese and Western Medicine on nonarteritic anterior ischemic optic neuropathy(NAION).METHODS Database as CNKI,WanFang Data,VIP and PubMed were searched for randomized controlled trials(RCTs)of integrated Traditional Chinese and Western Medicine on NAION from Jan.2010 to Feb.2019.Two researchers independently screened literature,extracted data and evaluated the risk of bias of included studies.Then,meta-analysis was conducted by RevMan 5.3 software.RESULTS A total of 10 RCTs were included,involving 733 patients.The results of meta-analysis showed that compared with Western Medicine alone,the integrated Traditional Chinese and Western Medicine in the treatment of NAION significantly improved central visual acuity(MD=0.08,95%CI[0.06,0.11],Z=6.03,P<0.01),mean light sensitivity(MD=2.19,95%CI[1.55,2.84],Z=6.66,P<0.01),total effective rate(OR=4.70,95%CI[2.90,7.64],Z=6.26,P<0.01)and reduced mean deviation(MD=-2.64,95%CI[-3.71,-1.58],Z=4.86,P<0.01).The difference between the two groups was statistically significant(P<0.01).CONCLUSIONS There evidence indicated that the clinical efficacy of integrated Traditional Chinese and Western Medicine in the treatment of NAION was significantly better than that of Western Medicine alone,especially in improving visual acuity and alleviating visual field defect.
